Note as dramatic as the "ghost" notes, but still very interesting.
Since I have never heard of an actual double denomination Canadian note, this is the closest there is. As to how it happened, it is thought that perhaps the sheet of paper that this note was printed on, may have been used to set up a press and obviously before a run of $100.
When the notes come into contact with each other, some of the ink can be transferred to the other note.

Basically, ghost notes are softly played notes, often played on the snare, that fill in the open spaces of grooves, for example you can play triplettes with the 1 being play on the hats and the 2 and 3 being played softly on the snare and using that as a connective tissue for a groove.
The ghost notes are all of those unaccented snare hits that are not on 2 and 4.
Ghost notes are those beats in between beats that are made to fill in the gaps.

Now if there is 2 notes in a BAR of 3/4...then it will be wrong to write that in note...right ?...then what we do is...enter a Ghost Note to fill up the 3rd note...but it shouldn't be played....
So it will be considered as 2 notes.Plus 1 note as Crotchet.So it comes to 5 notes,but will be regarded as 3 notes in a BAR of 3/4 beat song.
