  A Primer on Gleason Grading in Prostate Needle Biopsies
The Gleason grading system, based on prospective study of more than 4000 patients between 1960 and 1975, is the standard method of grading prostate cancer throughout the world.
Gleason patterns 1 and 5 are the least common patterns of prostate cancer.
Thus, assigning a Gleason score 2-4 to small foci of cancer in needle biopsy may give a false sense of security to the urologist and the patient and may result in inappropriate action such as watchful waiting.

Gleason has provided a conceptual diagram (oversimplified) in Figure 1 to show the continuum of deteriorating cancer cell architecture, and the four dividing lines along this continuum which he discovered are able to identify patients with significantly different prognosis derived from a study which included 2,900 patients.
Gleason Grade 4: This is probably the most important grade because it is fairly common and because of the fact that if a lot of it is present, patient prognosis is usually (but not always) worsened by a considerable degree.
By combining the patient's Gleason score with his PSA level and the clinical stage estimated by the physician, it is possible to use the Partin coefficient tables to estimate the likelihood that that patient has localized or locally advanced prostate cancer of different types.

Gleason Grade 5 means that the cells have lost most or all of their normal characteristics are very poorly differentiated and have essentially merged together into cancerous islands of cells.
The final Gleason Grade is the combination of two numbers, derived from adding the two highest grades assigned to two tissue areas extracted during the biopsy.  The pathologist will assign patterns to each section of tissue samples, a most common pattern in one sample and a second most common pattern in another specimen.

Gleason grading should be applied to all positive biopsies irrespective of the amount of cancer.
