  Data Sheet
Glyceryl trinitrate is a smooth muscle relaxant with vascular effects manifested predominantly by venous dilation with pooling.
The major beneficial effect of glyceryl trinitrate in angina pectoris is a reduction in myocardial oxygen consumption secondary to vascular dilation with resultant reduction in cardiac preload and afterload.
Continuous glyceryl trinitrate infusion at 3.1 to 4.0 mg/hr for 2-4 weeks in 36 patients resulted in an average methaemoglobin level of 0.2%, which was comparable to the level observed in patients receiving placebo.

Glyceryl trinitrate treatment consisted of an initial 10-mg patch, followed by a second 10-mg patch one hour later if there was no reduction in the frequency or strength of contractions.
Headache occurred in 30 percent of women in the glyceryl trinitrate group, while tachycardia (58 percent), palpitations (13 percent) and nausea (10 percent) were common in the ritodrine group.
Glyceryl trinitrate was also easier to administer and had a more benign side effect profile than ritodrine.

When glyceryl trinitrate was given sublingually peak plasma concentrations appeared within 4 minutes and at least half of the intact glyceryl trinitrate was cleared from the blood in 1 to 3 minutes (Reynolds, 1982).

Glyceryl trinitrate is taken as a tablet that is dissolved under the tongue, or as a spray that is sprayed under the tongue.
Glyceryl trinitrate taken in this way is used to give rapid relief from an angina attack, and can also be used to prevent an anticipated attack, for example before exercise.
Glyceryl trinitrate is also available as long acting tablets, skin patches and ointment that are used regularly each day to help prevent angina attacks.

The figure of nearly $28 million quoted as the cost to government for glyceryl trinitrate during 1992 refers to all the drug's forms and strengths that are listed on the Pharmaceutical Benefits Scheme.
The Committee recognised that there may be instances where patients do not require as many as 100 tablets, but it was considered that the minimal cost savings achieved by reducing the maximum quantity does not warrant the inconvenience or potential danger to patients who use the drug more frequently.
Thus, there would be minimal savings if glyceryl trinitrate tablets were packaged in smaller quantities and the availability of a smaller pack in addition to the 100 would mean unnecessary additional space in pharmacy dispensaries and wholesalers' shelves.

In general, three active substances are available for the treatment of an acute attack: glyceryl trinitrate (nitroglycerin), isosorbide mononitrate and isosorbide dinitrate (Table 4).
In this case the active substance for your P-drug of choice for an attack of angina pectoris would be: sublingual tablets of glyceryl trinitrate 1 mg.
In general only a small supply of glyceryl trinitrate tablets should be prescribed as the active substance is rather volatile and the tablet may become ineffective after some time.
