Leira, goddess of deception and illusions, was slain by Cyric with Godsbane and her portfolio absorbed by him.
Bane was destroyed by Torm during a battle in Tantras, and his portfolio was given to Cyric by Ao.

 Beat hades, no Epic unlock? - Titan Quest Forums
I beat hades today on normal with my elementalist.
Quest was updated, showing I defeated him, talked to persephone about how I'm the godsbane now, etc etc. Looted hades, went through the portal, credits roll....and epic is still grayed out.
So I reboot it, open TQ:IT again, go from my last save spot, make sure that the quest shows as finished, then go take on hades again to be sure.
