The Cagayan State University, also known locally as simply CSU, is the second largest state institution of higher learning in the Cagayan Valley Region, in terms of enrollment and number of curricular program offerings.
The University was established through Presidential Decree 1436, later amended by Republic Act 8292 by the integration of all publicly supported higher education institutions in the province of Cagayan.
In March 2004, Dr. Bong T. Balisi, former executive director of the Commission on Higher Education (CHED), a native of Tuguegarao City, was elected by the CSU Board of Regents to serve as the CSU System's third president, starting March 16, 2004.

The merger of the Cagayan Valley College of Arts and Trades (CVCAT), located at the capital town of Tuguegarao, Cagayan, and the Northern Luzon State College of Agriculture (NORLUSCA) at the Western Town of Piat, Cagayan signalled the birth of CSU by virtue of the decree.
Initial arrangements between the administration of the CSU and the Cagayan Valley Regional Hospital (now the Cagayan Valley Medical Center) as a laboratory for Medical students were finalized.
The CSU maintains seven satellite Campuses strategically located in the three Congressional Districts of Cagayan, namely Aparri, Gonzaga and Lallo in the First District; Sanchez Mira, Lasam and at Piat in the Second District; and Tuguegarao in the Third District.

In the health sector, 3 DOH-retained hospitals, 3 provinces (Cagayan, Isabela and Quirino) and 3 cities/municipalities (Tuguegarao, Pedro Gonzaga in Cagayan, and San Mariano in Isabela) are provided with low-cost imported medicines under the government’s GMA Pharma 50 Program with an average price savings of 52.9%.
In education, a total of 55 school buildings in unserved barangays in Cagayan, Isabela, Nueva Vizcaya, and Quirino were completed or are due for completion by July 2003.
Likewise, a total of 3,451,640 textbooks were procured from June 2001 to June 2003 resulting in a 1:1 textbook-pupil ratio in Grades 1 to 6, and in the 1st and 2nd Year High School in Region 2, except for English 3 (Grade 3) with a 1:2 ratio.
