Sent to the shallow Strait of Malacca for her sixth patrol, beginning in March, Grenadier was attacked by Japanese aircraft while approaching a convoy on 21 April and badly damaged by a bomb explosion after submerging.
Grenadier's entire complement was picked up by the Japanese, who employed prolonged torture in an unsuccessful effort to extract intelligence information.

Pre-dawn on 21 April, 1943, USS GRENADIER (SS 210) was making an end-around on two Japanese ships which had been sighted to the westward during the previous night.
Her Fairbanks-Morse engines were each delivering full power, driving GRENADIER along course 155 degrees toward Malacca Strait and a predetermined position from which it was anticipated a successful torpedo attack could be made on the two ships.
All tanks that would hold water had been filled in order to anchor GRENADIER and keep her from moving in the fast current of Malacca, and sliding off our perch and into deep water which was close by.

Patrolled 28 days, used 86% of the full, Grenadier will need at least 2 days to repair the 10% damage to the hull.
Although the revised depth settings may help some, we are getting a lot of premature exploding torpedoes and the rest of them duds bouncing off the hull of the enemy.

The Asst Navigator, an ETCS (SS) received a similar letter and was stripped of his Navy Submarine Enlisted Qualification, which ousts him from the Submarine Force.
Sadly, Joseph Ashley MM2 (SS) unfortunately suffered severe head injuries and was the only crew member to pass away.
