Because the isolation or identification of group A streptococci from a throat swab does not distinguish between a person with acute streptococcal infection and a person who is a streptococcal carrier, the best evidence of an antecedent streptococcal infection is a serologic response to the organism.

The group A streptococcus bacterium (Streptococcus pyogenes, or GAS) is a form of Streptococcus bacteria responsible for most cases of streptococcal illness.
All severe GAS infections may lead to shock, multisystem organ failure, and death.

Group A streptococcal (strep) infections are caused by group A streptococcus, a bacterium responsible for a variety of health problems.
Impetigo is an infection of the top layers of the skin and is most common among children ages 2 to 6 years.
The infection is spread by direct contact with wounds or sores or nasal discharge from an infected person.

Group A streptococcus is a bacterium often found in the throat and on the skin.
These infections are termed "invasive GAS disease." Two of the most severe, but least common, forms of invasive GAS disease are necrotizing fasciitis and Streptococcal Toxic Shock Syndrome.
Streptococcal toxic shock syndrome (STSS), causes blood pressure to drop rapidly and organs (e.g., kidney, liver, lungs) to fail.

 Group A Streptococcal Infections
Group A streptococci (GAS) bacteria are common and can cause a number of conditions ranging from sore throat to potentially fatal toxic shock or destruction of tissue.
Necrotizing fasciitis is a condition where muscle and fat tissue are broken down by enzymes that certain strains of group A streptococci produce, with a fatality rate between 20-30%.
During outbreaks, or if there is an indication of acute rheumatic fever, kidney infection, streptococcal toxic shock, or other complication, asymptomatic household contacts should be cultured.

Clinical Trial: Long Term Follow-Up of Patients With Group A Streptococcal Infection Originating From the Genital Tract
Invasive GAS infection is defined by the isolation of GAS from a normally sterile site (e.g., blood) or by the isolation of GAS from a nonsterile site in the presence of the streptococcal toxic shock syndrome or necrotizing fasciitis.
Because of the burden and severity of invasive GAS infection, the Centers for Disease Control and Prevention (CDC) hosted a meeting in October 2000 to formulate recommendations for the control of the disease among household contacts of persons with invasive GAS infections and to formulate guidelines for responding to postpartum and postsurgical GAS infections.
As some of these patients had severe infections (sometimes life threatening) a protocol for long-term follow up and management is necessary in case an invasive procedure is done (IUD insertion, endometrial biopsy, curettage or delivery) in order to prevent recurrent infection.
