You may almost always use the first form; you shouldn't if either there's a Jr part or the Last part has multiple tokens but there's no von part.
For example the First part of Per Brinch~Hansen's name has the single token "Per"; the Last part has two, "Brinch" and "Hansen"; and the von and Jr parts are empty.

Harbison and Steele have now gone through four editions and are beginning to cover language differences which can surprise the experienced C coder moving to C++.
You can find bigger books about C, but you won't find one as authoritative or helpful as this reference manual.
As always, the authors do an excellent job of explaining what's standard and what it replaces.
