HCM, or Hypertrophic Cardiomyopathy, is a heart disorder in which the left wall of the heart grows and thickens.
Symptoms of HCM are most frequently seen in young and middle-aged male cats, usually before the age of 5-6, but have been recorded in cats as young as 8 months and as old as 16.
The symptoms of HCM vary greatly, from cats that are seemingly unaffected, to cats that die suddenly.

HCMA :: Hypertrophic Cardiomyopathy Association | HCM Overview | What is Hypertrophic Cardiomyopathy (HCM)?
HCM is a primary and usually familial cardiac disorder with heterogeneous expression, unique pathophysiology, and a diverse clinical course, for which several disease causing mutations in the genes encoding proteins of the cardiac sacomere have been reported.
While HCM has typically been recognized by its structure ie., hypertrophy, the electrical function of the heart are also adversely affected.
The most characteristic feature of HCM is a hypertrophied left ventricle (asymmetric thickening of the wall usually most prominently involving the ventricular septum) without abnormal enlargement of the ventricular cavities.

Hypertrophic Cardiomyopathy Center - Background Data
Because HCM is a genetic disease, the clinical manifestations are varied and range from asymptomatic to congestive heart failure symptoms, stroke and even sudden cardiac death.
In fact, HCM is the most common cause of sudden cardiac death in the young (including trained athletes) while also being responsible for cardiovascular disability (heart failure and stroke-related complications) at any age.
HCM is a disease primarily of an abnormal enlargement of heart muscle (cardiac hypertrophy).

 Hypertrophic Cardiomyopathy (HCM)
Fundamentally, HCM is a genetic disease with autosomal dominant inheritance occurring in at least one in 1,000 to one in 500 of the general population.
For example, midcavity HCM is frequently a result of mutations in the essential or regulatory light chain (or the Leu908Val mutation of beta-myosin, in the domain that binds light chains).
In HCM as in many other genetic diseases, the basic hurdle is that any of a large number of different mutations in a large number of different genes may be causative.

eMedicine - Cardiomyopathy, Hypertrophic : Article by Michael E Zevitz, MD
Obstructive HCM is due to midsystolic obstruction to flow through the LV outflow tract as a result of a Bernoulli effect–induced systolic anterior mitral valve movement toward the septum.
The molecular basis for HCM is defects in several of the genes encoding for the sarcomeric proteins, such as myosin heavy chain, actin, tropomyosin, and titin.
The hallmark of HCM associated with a pressure gradient is the abnormal systolic motion of the anterior leaflet of the mitral valve (ie, systolic anterior motion) and, in rare cases, the systolic motion of the posterior leaflet.

That mitral regurgitation is very common in cats with HCM is clear, and this hemodynamic abnormality seems to be especially common when the ventricular septum is hypertrophied either symmetrically or asymmetrically.
When HCM is associated with aortic thrombosis, severe mitral regurgitation, atrial fibrillation, or biventricular heart failure with pleural effusion or chylothorax, the prognosis is guarded to poor.

HCM may also cause thickening in other parts of the heart muscle, such as the bottom of the heart called the apex, right ventricle, or throughout the entire left ventricle.
HCM may be inherited, caused by an abnormality in the gene that codes the characteristics for the heart muscle.
Because the cause of HCM varies, it is frequently difficult to identify a high-risk population.

The magnitude of this particular problem among all HCM patients has probably been exaggerated over the years due to the disproportionate number of reports from institutions in which there was the preferential referral of high-risk patients.
Therefore, an important aspiration of the HCM evaluation is to identify which patients, among all those with this condition appear to be at particularly high risk.
The latter disease feature applies to an important ten percent of all HCM patients in whom the maximum thickness of the left ventricular wall is 3.0 centimeters or more, and who may therefore be at increased risk based solely on their particular heart structure.

HCM has a complex set of symptoms and potentially devastating consequences for both patients and their families.
Thus, familial HCM is a genetically heterogenous disease because it is caused by defects at multiple genetic loci.
Paramount to any therapy for an individual with HCM is reduction in the risk of sudden death (by early identification of the disorder), effective medical care, and implantation of either a pacemaker or an automatic cardioverter defibrillator.

Hypertrophic Cardiomyopathy (HCM): Adult Onset Genetic Disease Program at The University Hospital, Newark, NJ
HCM scrambles the cells of the heart muscles, which can interrupt the heart’s electrical impulses and cause arrhythmia.
The test is done in two panels, one for the five most common mutations, the second for another three genes; together, the panels have a 55 percent to 70 percent detection rate.
Because HCM is an autosomal dominant disorder, it is important for all first degree relatives of an HCM patient to be tested for the gene mutation.
