HMS Sheffield, a Type 42 destroyer worth millions of pounds, was lost during the Falklands war......more importantly, 20 sailors died as a result — yet the MoD did not convene a court martial where witnesses and evidence in regard to the attack and subsequent sinking could have been properly examined.
Former crew allege that before the Inquiry they were told to alter their testimony so that evidence relating to the absence of a key officer from the ops room was removed.

 Adrian Nash - Falklands Range
Purists may note a few errors and omissions.
HM Ships 'Apollo', 'Bacchante', 'Brecon', 'Danae', 'Lowestoft' and 'Rhyl' were not directly involved in the fighting, whilst 'Leeds Castle' (OPV), and the 'Ambulance Carriers' (converted Survey Ships) 'Herald' and 'Hydra' were involved but were not modelled.
As commented on the previous 'Adrian Nash' page, these models should be considered extremely rare.
