The vaccine, called Gardasil, targets human papillomavirus (HPV) types 16 and 18, thought to cause 70% of cervical cancers, and HPV types 6 and 11, associated with 90% of genital warts cases.
In April, the researchers suggested that the vaccine might be most effective in 10- to 13-year-olds, who are not likely to be infected with HPV.
None of the females who received the vaccine was found to have precancerous changes or cervical cancer, compared with 21 of those who received the dummy shots.

Although HPV is widespread, cervical cancer is often caught in its earliest stages and is not a leading killer.
The final-stage study of the vaccine included 10,559 sexually active women ages 16 to 26 in the United States and 12 other countries who were not infected with the HPV strains 16 or 18.
Other types of HPV, which is sexually transmitted, also can cause cervical cancer and painful genital warts.

A vaccine that prevented the HPV infections known to be behind most cervical cancers would be a powerful addition to disease prevention strategies.
vaccine prevented women from becoming persistently infected with a virus that is associated with half of all cervical cancers, researchers reported in the November 21, 2002, issue of the New England Journal of Medicine (see the journal abstract of the study).
There are other efforts to develop a cervical cancer vaccine, as well, including one trial sponsored by NCI that is not yet open to enrollment.

NBC10.com - HealthWatch - FDA Examines Cancer Vaccine From Merck
The second concern is that any advantage the vaccine provides in protecting against the four virus types could be offset by infection by any of the multiple other types of HPV that the vaccine does not cover, according to the FDA documents.
The first is that the vaccine may lead to an increased number of cases of a cancer precursor among patients already infected by any of the four virus types at the time they receive the vaccine, and whose immune systems have not cleared the virus from their bodies.
Merck said the vaccine has the potential to reduce the annual number of new cervical cancers worldwide to roughly 150,000, from the current 500,000, and cut global deaths from the cancer by more than two-thirds, to an estimated 90,000.

Vaccine critics assert that the few studies on the cancer causing effects of SV40 are scientifically flawed, and that health officials have downplayed the potential risks since finding, in 1961, that the contaminant caused tumors in rodents.
Though Salk's vaccine had reduced the incidence of polio to a tiny fraction of what it was in the early 1950s, Sabin's vaccine was considered superior for these reasons and became the standard treatment.

We developed disease-transmission models for HPV-16 and HVP-18, the HPV types associated with most cervical cancer cases and the most likely to be included in HPV vaccines.
To capture the effect of a male vaccination program on female human papillomavirus (HPV) infection rates and cervical cancer incidence, we needed to directly model the effect of vaccination on HPV disease-transmission dynamics.

The demonstrated efficacy of an HPV 16 vaccine in preventing persistent infection in the female genital tract15 has raised expectations that such vaccines may eventually reduce the incidence of head and neck cancer.
However, the potential of vaccine programs designed for the genital tract to reduce the incidence of HPV infection in the head and neck is still to be evaluated.
Screening of head and neck cancers for HPV 16 and/or raised p16 as a marker of HPV-related transformation (Box) may identify a new group of patients (those with HPV-negative tumours) who would benefit from closer-than-usual follow-up or additional therapy.
