In several different types of games, hack and slash refers to a type of game or a style of gameplay which primarily comprises defeating enemies and monsters in combat, typically with swords or other melee weapons, hence the name.
In GNS Theory, "hack and slash" style is mostly associated with the "Gamist" archetype, due to its necessary reliance on numerous point-values and dice-rolling to simulate battles.
Hack and Slash were two separate fictional characters in ReBoot, the first computer generated TV series.

 Why D&D will never leave Hack -n- Slash
If D&D is considered hack and slash for having a lot of options for combat rules, and a listing on the character sheet..
Here's why I think the term Hack and slash is annoying: first of all, I don't care what anyone says: it's a very primitive style of roleplaying.
If there is a standard (of what causes a system to be or not to be 'hack & slash', and thus inherently primitive and thus suitable only to "beginners" or "weak roleplayers"), it should be standardized and applied to everything, or nothing at all.

Hack n' Slash fun has come to PSP in the form of the new title from Sony On-Line Entertainment (the people that brought us EverQuest II), Untold Legends: Brotherhood of the Blade.
The various monsters that rampage through the game are nicely animated and overall the title has pretty nice graphics for the hack n' slash adventurer.
Untold Legends: Brotherhood of the Blade is an awesome RPG game in he mold of the finest hack n' slash titles for the PC or PS2.

Today's Hack n' Slash is closer to the 'Beat em Up' genre than it is the classic 'Hack n' Slash'.

While that can be both a blessing and a curse, Dark Alliance is one of the better hack n’ slash titles available...but that’s basically all there is to it: hacking and slashing.
Sure, there’s some magic to be thrown in there, but then it just becomes hack n’ slash n’ burn.
This is what makes hacking and slashing so much fun in this title and puts it above all the rest.

Ever since the original Gauntlet was released in arcades, there has been a dedicated fanbase for hack 'n slash RPG's.
Fortunately, it is very easy to learn the controls, and the hack 'n slash gameplay is always fun.
This isn't the best hack 'n slash of all time (Phantasy Star Online holds that honor) but it is a very fun game, despite its repetitiveness.

Games become hack n slash because of 2 factors, lazy GM'ing and players with lack of imagination.
Rifts was designed specifically to get away from hack n slash and repetition by giving both GM and player a vast amount of gaming options.
Yeah sure thing about it getting pretty hack and slash but as a DM I let the players decide on which way a story unfolds sure I drop hints of under handed doings and politics into the picture and I always tell my players that you get more experience by thinking than just killing.

It wasn't a great game, but I'd have to second Rune; Diablo I and II were only called "hack and slash" games in my opinion because they're more action-oriented than the 3rd person isometric RPGs they resemble.
Rune is a true, literal hack and slash 3D fiasco -- you run and jump around in this stylized Nordic Viking land, swinging ridiculously large axes at other vikings and monsters, many of whom also wield ridiculously large axes.
It's not really hack and slash (unless you use the chainsaw throughout...), but if it's action you're lookin' for, it fits the bill quite nicely.

Sure, the hack ‘n slash got old after awhile, but who doesn’t appreciate some old-school hacking action and a couple of sore thumbs?
What Return of the King is as a game, is basically what The Two Towers should’ve been; that is, the improvements made to the first game’s formula are fairly small technically, but in terms of gameplay, change the whole dramatically, especially regarding the fun factor.
Just slashing away with the “X” button is just as effective, and also leaves you less vulnerable.
