HAEMORRHAGE - LoveToKnow Article on HAEMORRHAGE   (Site not responding. Last check: 2007-10-08)
Natural arrest of haemorrhage arises from (I) the coagulation of the blood itself, (2) the diminution of the hearts action as in fainting, (3) changes taking place in the cut vessel causing its retraction and contraction.
Haemorrhage has been classified as(r) primary, occurring at the time of the injury; (2) reactionary, or within twenty-four hours of the accident, during the stage of reaction; (3) secondary, occurring at a later period and caused by faulty application of a ligature or septic condition of the wound.
In severe haemorrhage, as from the division of a large artery, the patient may collapse and death ensue from syncope.

 Cerebral haemorrhage
This may be caused by cerebral haemorrhage (haemorrhagic stroke) or by blockage of an artery (ischaemic stroke).
Approximately one in 10 strokes is caused by cerebral haemorrhage.
A cerebral haemorrhage is bleeding from a ruptured blood vessel in the brain.

BBC - Health - Conditions - Subarachnoid haemorrhage
The haemorrhage that occurs when a berry aneurysm bursts is known as a subarachnoid haemorrhage because of the layer around the brain in which the bleeding occurs.
Not every brain haemorrhage is due to a berry aneurysm (although about 70 per cent of subarachnoid haemorrhages are caused this way, no defect can be found in at least 10 per cent, and there are other types of brain haemorrhage).
Of course, a stroke at any age is a tragedy, but a subarachnoid haemorrhage is a particular disaster because it is often fatal (as many as 30 per cent die within hours, and a further 30 per cent die within the first month) or results in serious disability in people who have young dependents.

 Subarachnoid haemorrhage
A subarachnoid haemorrhage is any bleed located underneath one of the protective layers of the brain known as the arachnoid layer.
A haemorrhagic stroke is caused by a ruptured blood vessel.
The two types of haemorrhagic stroke include the subarachnoid haemorrhage (bleeding in the space around the brain) and the intracerebral haemorrhage (bleeding within the brain tissue).

Haemorrhage in the germinal matrix at the level of the caudate nucleus or the hypothalamus is common in VLBW infants (incidence ± 45%).
Periventricular haemorrhage is graded according to the extent of the bleeding seen on scanning.
Extensive haemorrhages (grade III and IV) have a high mortality and morbidity as they are commonly associated with ischaemic brain damage.

Objectives To assess the incidence of gastrointestinal haemorrhage associated with long term aspirin therapy and to determine the effect of dose reduction and formulation on the incidence of such haemorrhage.
Results Gastrointestinal haemorrhage occurred in 2.47% of patients taking aspirin compared with 1.42% taking placebo (odds ratio 1.68; 95% confidence interval 1.51 to 1.881; the number needed to harm was 106 (82 to 140) based on an average of 28 months' therapy.
The pooled odds ratio for gastrointestinal haemorrhage with aspirin was 1.68 (95% confidence interval 1.51 to 1.88; P [is less than] 0.0001), and the number needed to harm based on an average of 28 months of aspirin was 106 (82 to 140).

Fatal haemorrhage following male ritual circumcision
It is suggested that the previously undiagnosed hepatic condition was responsible for the fatal haemorrhage.
We present the only case as far as we are aware, reported in the medical literature, of fatal haemorrhage due to a surgical misadventure in the course of a circumcision, in a baby affected by idiopathic neonatal hepatitis – a heterogeneous hepato-biliary disorder of undetermined cause.
Furthermore family physicians, neonatologists, obstetricians and paramedical staff, who are in contact with parents who desire to have their newborn circumcised, should advise them to request an expert and in case of any complications to report to a medical practitioner.

Stress preconditioning attenuates oxidative injury to the alveolar epithelium of the lung following haemorrhage in rats ...
The rats were haemorrhaged and resuscitated in order to determine the effect of haemorrhagic shock and fluid resuscitation on fluid transport across the lung epithelium.
Haemorrhagic shock was associated with a failure of the alveolar epithelium to increase vectorial fluid transport in response to catecholamines, despite the absence of any increase in the alveolar epithelial permeability to protein.
Finally, haemorrhagic shock was associated with a small increase in lung endothelial permeability to protein, as indicated by the significant increase in extravascular plasma equivalents of non-instilled lung in haemorrhaged rats compared with controls (Table 4).

When postpartum haemorrhage occurs, a number of medical and surgical interventions are used to control the bleeding [8].
If not, its use should be discouraged because of the dangers of side-effects, and the risks associated with widespread introduction of misoprostol into health systems in which it might be used for labour induction in inappropriate doses, with the risk of fetal compromise and uterine rupture [27,28].
In 6 observational studies [19-24], 41 women with postpartum haemorrhage were treated with misoprostol 1,000 μg rectally (32 women), 200 μg rectally (5 women), 200 μg orally 2-hourly (2 women) or 800 μg intrauterine (2 women).

Background: Primary postpartum haemorrhage is one of the top five causes of maternal mortality in both developed and developing countries.
It was not large enough to evaluate the effects of rectal misoprostol on maternal mortality, serious maternal morbidity or hysterectomy rates in women with primary postpartum haemorrhage.
Compared with a combination of intramuscular syntometrine injection and oxytocin infusion, rectal misoprostol administration showed a statistically significant reduction in the number of women who continued to bleed after the intervention and those who required medical co-interventions to control the bleeding (6% versus 34%) (relative risk 0.18, 95% confidence interval 0.04 to 0.67).

Relatives Of Patients With Subarachnoid Haemorrhage At Increased Risk   (Site not responding. Last check: 2007-10-08)
Dr David Gaist from Odense University in Denmark and colleagues investigated records of over 9000 (9367) patients in Denmark who were admitted to hospital between 1977 and 1995 with subarachnoid haemorrhage (a type of brain haemorrhage in which blood from a ruptured blood vessel spread over the surface of the brain).
Having identified individuals who had suffered subarachnoid haemorrhage, the authors then conducted follow-up research to ascertain whether any first degree relatives (parents, siblings or children) had also experienced such a haemorrhage.
Gaist et al found that first degree relatives of patients who have experienced subarachnoid haemorrhage are at a threefold to fivefold increased risk of experiencing such a haemorrhage themselves compared with the general population, but that the incidence rate of subarachnoid haemorrhage is low.
