Much of this is based in improvisation taught from a perspective that there are a certain set of "improv rules" that, should you follow, will lead to good improv.
As a result, improvisers tend to enter scenes with an inherent set of insecurities that, should they do a negotiation, transaction or teaching scene, or maybe ask a question, they have broken a "rule" and somehow failed.
Expectations in improvisation, and the resulting "failure" come solely from the instructor's idea of what he or she would have done with the scene, how he or she would have "written" it.

The “Harold” -- so named at the suggestion of one of the actors who pioneered the technique -- was created in the 1960s by Del Close, director of Second City, the renowned comedy group in Chicago that launched the careers of John Belushi, Bill Murray, Mike Myers and others.

I respectfully disagree with the idea that the Harold is flawed as a form.
The quality of the improvisation depends on the basic listening, reacting and yes anding of the players.
It was very hard for them when the more experienced players would hot dog, play for laughs, not listen, plan and deny their way through the evening.
