Heliox is a gas that is composed of a mixture of helium (He) and oxygen (O2).
Heliox has been used in a medical context since the 1930s, and although the medical community adopted its use initially to alleviate the symptoms of upper airway obstruction, its range of medical uses has since expanded greatly, most of which are dependent on the low density of heliox.
This is why Heliox is used in the alleviation of many medical conditions that involve a decrease in airway diameter (and consequently increased airway resistance), such as upper airway obstruction, asthma, chronic obstructive pulmonary disease (COPD), bronchiolitis and croup.

The beneficial effects of heliox are felt to be related to the lower density of helium, which preserves laminar flow at higher flow rates, decreasing work of breathing by decreasing resistance to airflow.

Although the beneficial effects of HELIOX were studied more than 60 years ago, this is the first report of its use in treating children with asthma.
Because HELIOX is about one-third as dense as air, it is thought to reduce the effort required to breathe through a narrowed airway.
HELIOX has been standard therapy for severe asthmatics in the pediatric and adult emergency rooms at the University of Chicago for nearly five years.

Heliox treatment duration ranged from 15 minutes to 384 hours, with an overall mean of 29.1 hours, with responders averaging 36.4 hours (median 16.8 hours) and non responders averaging 9.5 hours (median 4.5 hours)
This study suggests that in appropriately chosen patient populations, heliox is a useful adjunct therapy and the response rates are good.

The study, "Effect of heliox on lung dynamic hyperinflation, dyspnea, and exercise endurance capacity in COPD patients," was conducted by Paolo Palange, Gabriele Valli, Paolo Onorati, Rosa Antonucci, Patrizia Paoletti, Alessia Rosato, Felice Manfredi, and Pietro Serra from Dipartmento di Medicina Clinica, Servizio di Fisiopatologia Respiratoria, Università "La Sapienza," Rome, Italy.
The authors conclude that "heliox breathing, by reducing airflow limitations, lung dynamic hyperinflation and dyspnea sensation, is capable of improving high-intensity exercise endurance capacity in moderate to severe COPD patients." However, they note that "further studies are needed to verify the potential role of heliox supplementation during exercise rehabilitation programs in COPD patients."

Researchers from the University of Chicago in Illinois studied the spirometric effects of albuterol nebulized with heliox during emergency room visits by 45 patients with asthma.
Twenty-three patients were randomised to albuterol nebulized with heliox and 22 patients to albuterol nebulized with oxygen.
After the second treatment, it was 51.5 percent in the heliox group and 22.7 percent in the controls.

Heliox is widely used as a breathing mixture for deep-sea diving for reasons that are well established: when the nitrogen component of air is completely replaced by helium, this overcomes the narcotic effect caused by breathing nitrogen at the pressures prevalent at 50 metres’ depth and more.
