Topic: Hemoptysis

  Postgraduate Medicine: Manifestations of hemoptysis
Tuberculosis is a leading cause of hemoptysis in developing countries and should be a differential diagnostic consideration in patients who are immigrants from countries with a high prevalence of this disease.
Massive hemoptysis in patients with lung cancer is typically caused by malignant invasion of central pulmonary vessels by large central tumors.
Hemoptysis is a frightening symptom for patients and often is a manifestation of significant underlying disease.

The hemoptysis that results is usually mild to moderate and may be associated with other symptoms, including unexplained weight loss, cough, purulent sputum (thick, opaque, yellowish white discharge), and a history of mycobacterial exposure.
The hemoptysis is usually mild and often associated with a purulent sputum (thick, opaque, yellowish-white discharge), low-grade fever and occasionally, wheezing.
Infarction is a relatively uncommon cause of hemoptysis.

Hemoptysis, particularly if it involves a large quantity of blood or is recurrent, is a frightening, potentially fatal event, requiring an immediate search for the cause and precise location of the bleeding.
Hemoptysis must be differentiated from hematemesis and from blood dripping into the tracheobronchial passages from the nose, mouth, or nasopharynx.
The prognosis for patients with cryptogenic hemoptysis is generally favorable, usually with resolution of bleeding within 6 mo of evaluation.

 Hemoptysis | Building Better Health
Hemoptysis is the coughing up of blood or bloody sputum from the lungs or airway.
Massive hemoptysis is defined as 200-600 mL of blood coughed up within a period of 24 hours or less.
In spite of the number of diagnostic tests, the cause of hemoptysis cannot be determined in 20-30% of cases.

 Hemoptysis - coughing up blood
Hemoptysis, the act of coughing up blood, is an important symptom since it frequently reflects serious underlying lung disease.
A possible exception is mild hemoptysis occurring in a patient with chronic bronchitis during an acute exacerbation.
Massive or life-threatening hemoptysis (bleeding at a rate of greater than 200 ml per day) constitutes an emergency with the major diagnostic objective being localizing the source of the bleeding so that emergent surgery to remove the bleeding site can be carried out.

 Hemoptysis Referral Guidelines
Hemoptysis is the coughing of blood from the lower respiratory tract below the vocal cords.
The many infectious causes of hemoptysis include acute bronchitis, flare-up of chronic bronchitis, pneumonia, anaerobic lung abscess and cavitary lung diseases associated with necrotizing bacteria, fungi or mycobacteria.
In most cases hemoptysis is associated with mucous secretions due to bronchitis (and a negative chest x-ray).

Hemoptysis - Index - Pulmonology   (Site not responding. Last check: 2007-10-25)
Classifying hemoptysis as mild or massive (some practitioners classify it as trivial, moderate, or massive) is somewhat arbitrary, since it is difficult to accurately quantify the amount of blood that a person spits up.
Hemoptysis is considered massive, or major, when there is so much blood that it interrupts breathing (generally more than about 200-240 mL, or about 1 cup, in 24 hours).
Additionally, hemoptysis tends to occur intermittently and recur sporadically, and there is no way to predict if patients with mild hemoptysis are at risk for massive hemoptysis.

 PCCU Volume 15
Although hemoptysis is a frequent and potentially life-threatening complication of aspergilloma, the pathogenesis of hemoptysis remains unclear.
In addition, the hemoptysis may be the result of associated tracheitis or bronchitis and not directly related to the aspergilloma itself.
Because life-threatening hemoptysis occurs in a minority of patients, it is inappropriate to subject all patients with aspergilloma to therapy, especially as therapy is often associated with significant morbidity and mortality.

Because of the seriousness of lung cancer, patients with hemoptysis should be evaluated for the likelihood of bronchogenic carcinoma.
The technique is sometimes used to diagnose vascular causes of hemoptysis (i.e., causes related to abnormalities or blockage in the blood vessels).

Periodic hemoptysis occurring in association with the menses (catamenial hemoptysis) is a rare condition.
At the time of hemoptysis, there was a slight infiltration shadow in the right lower lung field on the chest radiographs, but this shadow disappeared after the menses.
Catamenial hemoptysis is rare, with only 30 reported cases[1-26] in the English literature since Latters et al[1] first described cyclical hemoptysis associated with the menses due to pulmonary endometriosis (Table 1).

Because as little as 600 ml of hemoptysis per 24 hours can produce major alteration in gas exchange, it would be unusual for a patient to display moderate blood loss anemia (which implies chronicity) on the basis of hemoptysis alone.
Exsanguination from hemoptysis would be very rare; DIC could occur, but less commonly than critical impairment of gas exchange.
