It appears that heparin binding to platelet factor 4 causes a confomational change in the protein, rendering it antigenic.

HIT type I is thought to be a nonimmune reaction to heparin that results in direct platelet activation, aggregation, and thrombocytopenia.
However, HIT should be strongly suspected in any patient who develops thrombocytopenia and unexpected thrombotic complications while undergoing heparin therapy.
