In light of the previous history of rectal carcinoma, a PET study was requested to evaluate for metastatic disease prior to resection of the hepatic flexure lesion.
Diagnosis: Tubulovillous adenoma of the colon hepatic flexure.
A single focus of moderately increased activity was found in the area of the previously described sessile mass in the ascending colon hepatic flexure suspicious for malignancy.

 Text for Abdomen Module
It is ensheathed along with the common hepatic artery and common bile duct in the hepaticoduodenal ligament and form the portal triad entering through the portal hepatis.
It is related to the 12th rib posteriorly and to the liver, duodenum, and hepatic flexure of the colon anteriorly.
Another branch of the common hepatic artery is the proper hepatic artery, which supplies blood to the liver.

A colic flexure is a flexure (a 'bend') in the colon.
There are two colic flexures in the transverse colon.
The right colic flexure is adjacent to the liver, and is therefore also known as the hepatic flexure.

part of the large intestine that is continuous with the transverse colon at the left colic (splenic) flexure and the sigmoid colon at the pelvic brim
hepatic flexure lies anterior to the lower part of the right kidney and inferior to the right lobe of the liver; also known as: right colic flexure
transverse colon is continuous proximally with the ascending colon at the right colic (hepatic) flexure; it is continuous distally with the descending colon at the left colic (splenic) flexure; it is suspended from the posterior abdominal wall by the transverse mesocolon

After giving off the gastroduodenal artery, it becomes the proper hepatic artery, which divides into right and left hepatic arteries (thirty percent of the blood flow into the liver is through these arteries, the rest is from the portal vein).
It is suspended on a mesentery, the transverse mesocolon, from the posterior abdominal wall between hepatic and splenic flexures.
It extends from the splenic flexure to the sigmoid colon.

Extend the scissor incision upwards to the hepatic flexure of the colon.
The aim is to free the hepatic flexure of the colon and that part of the greater omentum which is attached to it.
It is also sensible to include the hepatic flexure in the resection in case the ligation of the right colic artery devascularises the hepatic flexure.

 Commonly Used Words and Definitions
Hepatic flexure: The right-angle bend in the colon on the right side of the body near the liver that marks the junction of the ascending colon and the transverse colon; also called right colic flexure.
Sigmoidopexy: Surgical attachment of the sigmoid flexure to the wall of the abdomen for relief of rectal prolapse.
Splenic flexure: The bend of the colon under the spleen where the transverse colon joins the descending colon; also called the left colic flexure.

HIDI Colonoscopy   (Site not responding. Last check: 2007-10-21)
Once the splenic flexure has been passed, the transverse colon is often easy to negotiate and the hepatic flexure is seen as an acute bend.
When the hepatic flexure is seen, air should be aspirated to collapse the flexure towards the tip of the colonscope, which should then be steered around the bend.
If difficulty is encountered in passing the hepatic flexure, and the patient is in the supine or right lateral position, then positioning the patient in the left lateral position will often help.

Continuing that line of thought, the hepatic veins are similar to the pulmonary veins in that they reside between lobes (segments) and drain blood from two or more adjoining areas.
Other uncommon variations include: cystic artery as branch of a normal proper hepatic artery (6%), cystic artery from left hepatic artery, crossing anterior to the bile duct(s) (3%), and cystic artery as an independent branch of the gastroduodenal artery, again crossing anterior to the duct (3%).
Hepatic veins are are formed by the union of the central veins of the liver and open into the IVC just inferior to the diaphragm.

Subsequent tagged RBC study demonstrated active bleeding within the colon at the hepatic flexure.
Delayed (60-90 minute) images demonstrate a site of active bleeding located at the hepatic flexure with activity increasing in intensity over time, peristalsing distally and also moving dependently in a retrograde fashion to pool in the cecum.
Only after evaluating the dynamic cine images does it become apparent that the bleeding originates in the region of the hepatic flexure and reaches the cecum through dependent pooling.

Re: IBS pain relief? I might have found it.
The reason this is so, IN MY OPINION, has to do with what is called the splenic flexure of our large intestine (left side) and hepatic flexure of our large intestine (right side).
What this means is that these two flexures are where our colons go from the horizontal position to the vertical position, one on the left by our spleen ("splenic" flexure) and one on the right by our liver ("hepatic" flexure).
These flexures are where there seems to be a lot of gas buildup in people with IBS.
