The concept of the hero in the game having to sneak around the bad guys instead of the typical idea of charging in, guns blazing was what the core on Metal Gear is. Hideo Kojima always had interest in America, and had a passion for filmmaking.
It was later released in America for the NES, but it had gone through many changes without Kojima overseeing it's english translation, so the more well-known version of the game for the NES isn't actually the original game that Kojima-san made.
Later, after the game's success, Hideo would go on to make the second installment in the Metal Gear sereis entitled: "Metal Gear 2: Solid Snake" This game was much darker and adult-oriented than the original, and had some of the best graphics in games thus far.

Of course this isn't the first time Kojima has tried to ditch his famous stealth series; originally MGS2 was going to be his final series entry but he reclaimed the mantle for the third game after massive fan outcry.
Yeah, as I recall, he said MGS3 would be the last one at one of the E3's before it was announced.
