A History of the Doctor Who Theme
Doctor Who was first broadcast on BBC television on 23rd November 1963, at 17.16hrs.
The third story of the 1970 season of Doctor Who was The Ambassadors of Death (originally referred to in Radiophonic Workshop documents by one of its working titles, Carriers of Death), and it was decided to use the adventure for some further experiments with the titles.
In November 1972, with transmission of The Three Doctors still a few weeks away, and the decision to drop the Delaware version of the theme still to be taken, a new "full-length" version of the music was commissioned, to be released as single by BBC Records in April 1973.

Amazon.com: A Critical History of Doctor Who on Television: Books: John Kenneth Muir
He begins with a cogent analysis of the origins of "Doctor Who," identifying H.G. Wells' The Time Machine (specifically, the 1960 George Pal film with Rod Taylor) and Nigel Kneale's marvelous "Quatermass" stories (produced by BBC TV in late 1950's) as the two templates around which the vast majority of "Doctor Who" stories are built.
It is not until well past his review of the show's history that he mentions, almost in passing, that "Doctor Who" ceased being a kiddie show by 1975.
His failure to grasp this essential point as the appropriate focus of a critical history -preferring instead to draw out lengthy parallels between "Doctor Who" and the many science fiction TV shows of the sixties and seventies - reduces the book to a catalogue of plots and themes rather than a critical history.
