Perhaps the most important aspect of convergence in human error research has been movement toward at least a partial model of human cognition consistent with a broad spectrum of human error research.
Human cognition uses processes that allow us to be amazingly fast [Reason, 1990], to respond flexibly to new situations [Reason, 1990], and to juggle several tasks at once [Flower and Hayes, 1980].
Although the emerging model of human error is compelling and fits a broad spectrum of research in a qualitative fashion, it cannot, at this stage, predict error commission rates or error correction rates.

Human Factors researchers and professionals have been and continue to be eager to join in such partnerships to make progress on patient safety.
Human factors can focus attention on underlying mechanisms, behavioral patterns, and contextual contributions and provide the methods and tools to design the support systems that match patient needs.
Judges in the group told of the negative outcome consistently assess the performance of humans in the story as being flawed in contrast with the group told that the outcome was successful.

Visual Expert Human Factors: Medical Error
In my view an error can occur only if there was or should have been an appropriate intention to act on the basis of a perceived or a remembered state of events; and if the action finally taken was not that which was or should have been intended.
Errors, to the extent that we have data, are random; the moment when an error will occur cannot be predicted.
The actor is the victim of the error; the patient is the victim of the expression of the error in a medical setting which permits the error to be completed and produce an injury.

System designers must insure that the HCI is easy and intuitive for human operators to use, but not so simple that it lulls the operator into a state of complacency and lowers his or her responsiveness to emergency situations.
However, if the human operator must routinely be involved in the control of the system, he or she will tend to make mistakes and adapt to the common mode of operation.
Humans have higher failure rates under high stress levels, but are more flexible in recovering from emergency situations and the last hope in a potential disaster.

Visual Expert Human Factors: Human Error in Road Accidents
A comprehensive study of road safety (Treat et al., 1977) found that human error was the sole cause in 57% of all accidents and was a contributing factor in over 90%.
Humans have limited information processing abilitiers and must rely on three fallible mental functions: perception, attention and memory.
When a driver fails to avoid an accident because the situation exceeds these these limitations, it is often called "human error." In reality, it is often the situation that is primarily responsible, not the driver's response to it.

 Human Error
Human error is almost always the direct cause of any consequential event.
The worst thing, however, is that pinning a problem or event solely on human error prevents you from finding and addressing a whole host of other factors that are probably more important.
Combining this concept with the realization that human error can never be completely eliminated, and it becomes very clear that we should be spending our time investigating and fixing the system; blaming a problem on human error is almost always a cop-out.

The types of errors we make in doing simple tasks for ourselves, especially in using devices, are the same types of errors we make in complex system design and operation.
We will try to avoid taking responsibility for errors because of Karchmer’s Law of Performance Evaluation: ten "attaboys" equals one "aw, shit." An engineer who hasn’t accumulated a substantial number of "attaboys" by the time the error is committed may be out of a job.
Human error can't be avoided because even if our ability to learn from our mistakes improves nearly to perfection, everything will change around us.

Human error is cited more often than mechanical defects in the roughly 5,000 truck-related deaths that occur each year on the nation’s roads.
“The overriding issue is one of driver distraction and whether the manufacturers are designing their technology in a responsible way,” said H. Keith Brewer, agency’s director of human centered research.
Pat Parker said he adopts the same outlook at his Grand Rapids trucking operation, which is why he’s managed to avoid the high turnover rates suffered by many other truck operators, who can see up to 100 percent turnover in a year.
