Lamotrigine: A Significant Advance for Treatment of Bipolar Depression
Hence, if a bipolar 1 depressed patient experiences a manic or hypomanic episode while on a given medication, the episode might have been triggered by the medication or the episode might have been the natural expression of the course of the disorder and the medication played no role.
The best way to ascertain whether a medication has a tendency to induce manic and hypomanic episodes in bipolar disorder patients is to examine the incidence of manic and hypomanic episodes in patients on the active medication in comparison to those on placebo.
The incidence of manic and hypomanic episodes was examined in bipolar 1 depressed patients on lamotrigine monotherapy, compared with those treated with placebo monotherapy for 7 weeks (2).

Longitudinal follow-up studies indicate that the risk of bipolar disorder developing in patients with cyclothymia is about 33 percent; although 33 times greater than that for the general population, this rate of risk still is too low to justify viewing cyclothymia as merely an early manifestation of bipolar type I disorder.

 THE HYPOMANIC EDGE -- Reviews
Cass claims that the majority of his patients are hypomanic, and though he treats them for the problems that hypomania can produce - depression, burnout, substance abuse, wrecked relationships - he also recognizes its advantages.
But instead of spiraling into debilitating manias and then into a paralyzing depression, hypomanics generally experience only the invigorating effects of the onset of mania and usually emerge from it without professional help (though sometimes a period of mild depression follows).
Indeed, while hypomania has been the subject of relatively little research and clinical attention, there are a handful of studies indicating that people in a hypomanic state are more flexible and creative in their thinking, are more motivated and productive, and have more positive expectations for the future.

Hypomania is a bit difficult to define, because the term seems to be used slightly differently by different doctors, and the criteria for the diagnosis are slightly different in ICD-10[1]and DSM-IV[2] (the reference books which give standard symptoms for the diagnosis of different conditions).
A manic episode differs from a hypomanic episode, in that, it is severe enough to have an adverse effect on a person’s work, relationships or usual social activities.
Although manic episodes and hypomanic episodes have identical lists of characteristic symptoms, the mood disturbance in hypomanic episodes is not sufficiently severe to cause marked impairment in social or occupational functioning or to require hospitalisation.
