Topic: Hypothermic

Hypothermia - Wikipedia, the free encyclopedia
Moving a severely hypothermic person can, and most likely will cause ventricular fibrillation (heart attack), so if possible wait for trained emergency workers arrive.
In a hospital, warming is accomplished by external techniques (blankets, warming devices) for mild hypothermia and by more invasive techniques such as warm intravenous fluids or even lavage (washing) of the bladder, stomach, chest and abdominal cavities with warmed fluids for severely hypothermic patients.
These patients are at high risk for arrhythmias (irregular heartbeats), and care must be taken to minimize jostling and other disturbances until they have been sufficiently warmed, as these arrhythmias are very difficult to treat while the victim is still cold.

 Hypothermia and cave rescue
Re-warming shock is associated with severely hypothermic patients and is associated with relative hypovolemia as a result of vasodilatation during re-warming and accompanying dehydration.
One of the most dangerous situations is for the severe to moderately hypothermic patient to be surface re-warmed only and the resulting shunt of cold blood to the core and release of metabolic wastes into the core are devastating to the patient who is already ill and in a threatened condition.
Hypothermic patients do not respond to advanced care in the same manner as normothermic patients and modifications must be made in the care offered to these patients.

Hypothermia, Prevention, Recognition, Treatment: Hypothermia Special Situations.
Passive rewarming methods, to be used in mildly hypothermic victims and as an adjunct in moderate-to-severe hypothermia, include heat packs to arms and groin areas, heating lamps, warmed blankets, and warm-air-heated "sleeping bag" devices.
The usual parameters for fluid assessment may be difficult to use in a hypothermic victim, due to large quantities of fluid in the "third space" and the clinical difficulty of obtaining orthostatic blood pressures and weight.
Hypothermic victims should be treated aggressively, because even when all vital signs are absent, survival without neurological impairment may be possible in certain patients.

Hypothermia, Prevention, Recognition, Treatment in Animals.
In our views, this information is likely to be a valuable in treating hypothermic animals, however, we advise practitioners to use this knowledge along with their experience in dealing with hypothermic patients.
In the hypothermic state, the oxygen need of cells, particularly neurons is greatly reduced, and the circulation can be stopped for relatively long periods.
The simplest way to determine whether the patient is hypothermic or not, is to assess body temperature by placing a bare hand against the skin (preferably in axilla or groin region) of the patient.

Hypothermic Circulatory Arrest Procedures for Giant Intracranial Aneurysms (Indications and Technical Aspects)   (Site not responding. Last check: 2007-10-22)
The use of deep hypothermic circulatory arrest as an adjunct during clipping complex giant aneurysms has shown promise as a cure for otherwise incurable lesions.
Deep hypothermic circulatory arrest is certainly not a new concept.
The key to successful outcome with deep hypothermic circulatory arrest surgery is to formulate a dedicated team that includes a neurosurgeon, a cardiac surgeon, an anesthesiologist experienced in both subspecialties, and pump technicians familiar with peripheral bypass.

Severely hypothermic patients may have other problems that are not easily detected by rescuers, but which may affect the patient's survival.
Moving a hypothermic patient's extremities may also cause cold peripheral blood to be pumped into the central circulation, affecting cardiac rhythm.
Most hypothermic patients are dehydrated, administer 300-500 cc's of dextrose in water or saline rapidly, followed by 75-100 cc/hr.

The hypothermic patient may appear to be beyond help because of skin color, pupil dilation, and depressed vital signs However, patients suffering from severe hypothermia have been resuscitated.
However, hypothermic patients may be more difficult to intubate, precipitation of ventricular fibrillation may be a danger, and intubation should only be attempted under optimum conditions by skilled personnel only if the airway is inadequate.

The study objective was to compare the clinical and physical characteristics of febrile septic patients with hypothermic septic patients.
The most important baseline differences in baseline characteristics within the hypothermic group (control vs. treated) were:1) age of almost 10 yrs between groups (p = 0.05) and 2) a difference in positive blood cultures, 50% in the ibuprofen group vs. 20% in the placebo group (p <.05).
The mortality rate in the hypothermic patients treated with Ibuprofen was significantly smaller than the hypothermic placebo group.

Moreover, all the hypothermic patients in the Shiozaki study had a temperature of less than 35 °C, while in our study only one patient had less than 35 °C and this was found in a pre-surgical hypothermic patient with sepsis.
This was possibly due to the small size of the sample, which was the result of the impossibility of evaluating a larger proportion of the patient population.
