 Changes to the Laboratory National Coverage Determination (NCD) Edit Software for October 2005
In accordance with the coding analysis, CMS is adding new ICD-9-CM codes to the list of ICD-9-CM Codes Not Covered by Medicare for the Negotiated Laboratory NCDs.
In accordance with the coding analysis, CMS is adding new ICD-9-CM code 585.6, End Stage Renal Disease, to the list of ICD-9-CM Codes Covered by Medicare for Urine Culture.
CMS is deleting ICD-9-CM codes, 287.3, Primary Thrombocytopenia, and 585, Chronic Renal Failure, from the same list for this NCD.

 Laboratory National Coverage Determination (NCD) – Changes
In Program Memorandum AB-03-104, CMS announced a number of ICD-9-CM codes that were deleted by the update in ICD-9-CM codes that became effective October 1, 2003.
The following diagnosis codes are deleted from the list of ICD-9-CM Codes covered by Medicare for PT and partial thromboplastin time (PTT) NCDs:
In Program Memorandum AB-03-104 (CR 2814), CMS announced the addition of diagnosis code 401.1, benign essential hypertension, to the list of covered diagnoses for lipid testing.

 WPS Medicare Part B - Clinical Lab Coverage Determinations Take Effect Nationally
The first list, "ICD-9-CM codes covered by Medicare," includes codes where there is a presumption of medical necessity, but the claim may be subject to review to determine whether the test was in fact reasonable and necessary.
The third list, "ICD-9-CM codes that do not support medical necessity," includes diagnoses that generally are not covered for the test but for which there are limited exceptions.
If a code from this section is given as the reason for the test, the test may be billed to the Medicare beneficiary by the lab without billing Medicare first.

 Medicare Newsroom: [Changes to Clinical Laboratory National Coverage Determinations]
As a result of the annual ICD-9-CM code update, the following changes are being made to the NCDs.
· In the urine culture bacterial NCD list of covered diagnoses, we are removing code 600.0 and replacing it with 600.00 and 600.01; removing code 600.1 and replacing it with 600.10 and 600.11; removing 600.2 and replacing it with 600.20 and 600.21; and removing 600.9 and replacing it with 600.90 and 600.91.
We are also removing code V04.8 and replacing it with V04.81, V04.82, and V04.89; removing V53.9 and replacing it with V53.90, V53.91 and V53.99; removing V54.0 and replacing it with V54.01, V54.02, and V54.09.

 Attachment I: Development of ICD-10-CM and ICD-10-PCS; Overview of ICD-9-CM
The Centers for Medicare and Medicaid Services (CMS), formerly the Health Care Financing Administration (HCFA), began developing a replacement to ICD-9-CM, Volume 3 in 1990 in order to address a number of limitations with the current procedure classification.  ICD-9-CM procedure codes are made up of four digits.
An updated pre-release draft of ICD-10-CM is posted on the NCHS web site at: http://cdc.gov/nchs/icd9.htm.  The files, in PDF (Adobe) format include the tabular list, alphabetic index, the external cause of injury index and the Table of Neoplasms, as well as draft coding guidelines.
There are only 10 codes available within each category (fourth digits 0-9).

Logical maps ensure that for a given ICD-10-AM code, the backward ICD-9-CM code and any ICD-9-CM codes which forward map to the code are in the same principal diagnosis list for the grouper.

As discussed below, the codes reported per ICD-10-CM chapter totaled 23, 122, but this figure may also be low.
Almost 10 percent (7.9%) were records from post-acute settings, which encompassed home health or hospice, nursing homes, long term care hospitals, and rehabilitation units or facilities.
Six percent (6%) of the coded records were from physician practices, with a small percentage of records from clinics, community health centers, freestanding ambulatory surgery centers, and freestanding diagnostic facilities (2.9%).

All codes in ICD-10-CM are alphanumeric, i.e., one letter followed by two numbers at the three-character level, as opposed to three numeric characters in the main classification of ICD-9-CM.
Codes in ICD-10-PCS contain seven alphanumeric characters, and although there are thousands of possible codes, PCS distills all medical services into about 30 procedures.
The index allows codes to be located alphabetically and is arranged by root operations with subentries by body system, body part, operation, and device.

 Modelling ICD-10-AM
Many countries use WHO ICD-10 diseases with ICD-9-CM procedures, some with ICPM translated or updated from its 1978 version.
If ICD classifications were modelled, the relationships between terms and classes would be more explicit, and inform processes such as mapping, new classification development and creation of specialised subsets of classifications, not to mention extraction of clinical terms from electronic health records for multiple uses.
