isoniazid, INH - Skin diseases, conditions and procedures on MedicineNet.com
Tuberculosis is an infectious diseased caused by a bacterium.
INH can increase the effect of certain benzodiazepines, such as diazepam (Valium), triazolam (Halcion), and others, by interfering with the enzymes in the liver that eliminate benzodiazepines.
INH can decrease the rate at which the liver eliminates phenytoin (Dilantin), which can raise the blood levels and result in side effects of phenytoin.

 OCC-ENV-MED-L archives -- August 2000 (#306)
Patients found to have INH monoresistant organisms after induction therapy is complete (e.g., during continuation phase of therapy with INH and rifampin) should be evaluated for treatment failure clinically and with cultures.
Patients who are identified as infected with INH monoresistant organisms before the induction phase of therapy is completed may be treated with a combination of rifampin, pyrazinamide, and ethambutol (or streptomycin) for 6 months.
Patients with unrecognized INH monoresistance who were treated with the two-drug regimen of INH and rifampin and those treated initially with INH, rifampin, and pyrazinamide are at increased risk for treatment failure and/or relapse after treatment, possibly associated with acquired rifampin resistance.

University of Miami School of Medicine - Glossary - isoniazid, INH
The mechanism of action of INH is not known, but it is thought to work through its effects on lipids (fats) and DNA within the tuberculosis bacterium.
It is best to take INH on an empty stomach for maximum absorption into the body, but if it causes abdominal discomfort, it may be taken with food or with a non-aluminum antacid such as Tums or Titralac.
Because rifampin can be toxic to the liver, the use of rifampin and INH together increases the risk of liver toxicity to a level that is greater than with either drug alone.

 Multidrug-Resistant Mycobacterium tuberculosis: Molecular Perspectives
INH and RIF, the two most potent antituberculous drugs, kill more than 99% of tubercule bacilli within 2 months of initiation of therapy (16,17).
Efforts to determine the factors involved in resistance to INH led to the discovery of the inhA locus, which was proposed as the primary target for coresistance to INH and ethionamide (54).
INH susceptibility could result from incorporation of iso-NAD, which is formed as a consequence of the action of KatG on INH, and thus hinders the enzymatic activity of InhA and blocking fatty acid synthesis (56).

Current recommendations that encourage wide use of preventive INH therapy are unwise becuase they inflict a risk of fatal hepatitis on compliant adults and older children who have little danger of tuberculosis while being difficult to deliver to the alcohol- and drug-addicted persons whose risk is high.
She had been given monthly allotments of INH and was well until the acute onset of nausea, vomiting, and jaundice on August 2, 1990.
Preventive INH therapy had been recommended despite her age because of "close contact and recent tuberculin conversion." Her physician could not document the date of a previous negative tuberculin test and she was not a household contact.

Their questionnaire data indicated that nearly two-thirds of physicians cited the reason for not starting INH chemoprophylaxis in these older patients was solely the result of "fear" of age-related side effects.
Since their study did not address how many cases of TB were specifically prevented, they obviously could not ascribe a benefit-risk profile of INH as did Stead and coworkers[6] in their population.
The benefits of INH chemoprophylaxis have been clearly delineated, especially in populations at risk, such as recent contacts of a known case or in recent converters.

Acquired C1 INH deficiency states were discussed in a lecture at the World Allergy Organization meeting in Vancouver, Canada.
Treatment approaches vary in congenital and acquired C1 INH deficiency: 1) Congenital (with either markedly reduced levels or normal levels but dysfunctional C1 INH) is best treated by altered androgen preparations (danazol, stanazolol).
Acquired C1 INH deficiency states can usually be differentiated from congenital C1 INH deficiency states by mearirement of the serum C1 levels (decreased in acquired, not in congenital).

Acquired angioedema (AA) variants of HAE are due to either: 1) immune reaction associated with certain lymphoproliferative disorders and malignancies that consume the C1 INH as a result of increased complement activity or 2) an anti-C1 INH antibody.
