In pratica l'utente che sottoscrive un abbonamento ISDN riceve un flusso dati digitali di 144 Kbit/s suddiviso in due canali fonia/dati da 64 Kbit/s (quindi due linee telefoniche indipendenti) e un canale di segnalazione, riservato alla segnalazione fra la centrale e le apparecchiature connesse, di 16 Kbit/s.
Il segnale ISDN non viene fornito direttamente all'apparecchio telefonico come avveniva con la telefonia analogica ma passa attraverso una borchia ISDN, un apparato che converte i dati di segnalazione e separa i due canali di fonia digitale smistandoli su altrettante prese, a cui andranno collegati (sempre tramite un collegamento digitale) gli apparecchi ISDN.
Il servizio ISDN in Italia viene fornito in modalità mononumero o multinumero: è possibile cioè richiedere fino a 8 numeri telefonici diversi.
