For such a small sports car the Ibuki rides on massive 18 inch wheels, these are pushed to the corners and sit under slightly extended wheel arches.

I hit Ibuki with a Tiamat's Rampage, and use the E from Original Style.
Ibuki takes 12 damage and loses 4 vitality from Original Style, Ibuki uses her R, Ibuki goes to full.
I lean towards the former, as it 'adds on' 4 vitality loss, as a card effect (much like causing a discard or committing a card), and would therefore resolve separately from attack damage, especially considering that vitality loss is separate from damage (and cannot be reduced the same way that damage can).
