  Incidentaloma
In medicine, an incidentaloma is a tumor (-oma) found by coincidence (incidental) without clinical symptoms and suspicion.
It is a common problem: up to 7% of all patients over 60 may harbor a benign growth, often of the adrenal gland, which is detected when diagnostic imaging is used for the analysis of unrelated symptoms.
Other organs that can harbor incidentalomas are the pituitary gland, the liver (often a hemangioma) and the kidneys.

Among 317 incidentalomas that were aspirated from 267 patients the rate of malignancy was 12% in a retrospective analysis.
These thyroid lesions should be managed like other “Incidentalomas”, with observation, dedicated thyroid sonography, aspiration biopsy, or even surgery, as indicated by the data and mature judgment.
Not all "incidentalomas" in the region are thyroid in origin.

Management of the Clinically Inapparent Adrenal Mass - September 15, 2003 - American Family Physician
When adrenal masses are discovered incidentally during diagnostic studies for other clinical conditions, they are commonly termed "incidentalomas." When an adrenal mass is discovered, management depends on determining whether the lesion is nonfunctioning or hormonally active and whether it is benign or malignant.
If an incidentaloma is nonfunctioning, variables that affect management include the size, imaging characteristics, and growth rate of the lesion.
Because of the complexity of the problem, the management of patients with adrenal incidentalomas is optimized by a multidisciplinary team approach involving physicians with expertise in endocrinology, radiology, surgery, and pathology.

An incidentaloma is a benign growth or tumor found by coincidence when a doctor is looking for something else.
The annoying part is they cause worry and may require expensive workups to show that, yes, it is a benign growth, and no, it is not a lucky discovery of early-stage cancer.
Incidentalomas are frequently found on the adrenal glands.

Management of Clinically Inapparent Adrenal Mass: Summary of Evidence Report/Technology Assessment No. 56
Management of adrenal incidentaloma is a complex decisionmaking process that involves considering a range of possible diagnoses and their natural history, diagnostic testing and interpretation, and weighing the risks and benefits of interventions in light of the patient's age and the tumor size.
Sixty percent of the incidentalomas occurred between the ages of 41 and 60, and 90 percent between the ages of 31 and 70.
A strict definition of incidentaloma would aid in the interpretation of results from clinical studies; however, it will not be sufficient to address the diverse manifestations of this condition that are also clinically relevant.

Hot Thyroidology - Journal owned by the European Thyroid Association
The problem of the thyroid incidentaloma is the problem of the clinical significance of an impalpable small thyroid nodule (usually less than 10 mm diameter and not more than 15 mm) discovered incidentally during investigation for another disease process or during screening tests on a well subject.
The term incidentaloma is a clinical one best used for the as yet undiagnosed nodule and if a malignancy is subsequently identified the term then to be used is microcarcinoma.
Kang et al (12) found a 2.4% prevalence of focal thyroid incidentalomas by PET scanning in a cancer screening program and a 1.6% prevalence in a combined group including subjects being evaluated for metastases from a non-thyroidal malignancy, indicating a high prevalence of incidentaloma although lower than that of both ultrasonographically-detected nodules and occult microcarcinoma.

Adrenal incidentaloma is a tumour that is usually discovered fortuitously during an imaging examination.
The term incidentaloma is used almost exclusively for tumours of the adrenal glands.
In the majority of cases, incidentalomas are asymptomatic but they may be associated with symptoms related to hormone hypersecretion (pheochromocytoma-like symptoms, hyperaldosteronism or Cushing syndrome).

 Disorders & Descriptions
An adrenal incidentaloma is a mass or tumor (larger than one centimeter) that is found on the adrenal gland (the gland located above each kidney).
The term "incidentaloma" refers to the fact that the mass is usually found "incidentally," that is, while the patient is being examined for another condition, such as abdominal pain or injury.
An adrenal incidentaloma is found in up to 5% of all people who are undergoing CT scans of the chest and abdomen, ultrasound of the abdomen, or magnetic resonance imaging (MRI) of the abdomen.

The low prevalence of adrenal cortical carcinomas and the relatively low incidence of progression to hyperfunction call into question the advisability of the current practice of intense, long-term clinical follow-up of this common condition.
Because of the complexity of the problem, the management of patients with adrenal incidentalomas will be optimized by a multidisciplinary team approach involving physicians with expertise in endocrinology, radiology, surgery, and pathology.
In patients with tumors that remain stable on two imaging studies carried out at least 6 months apart and do not exhibit hormonal hypersecretion over 4 years, further follow-up may not be warranted.

An adrenal incidentaloma is defined as a mass lesion found unexpectedly in an adrenal gland by an imaging procedure performed for reasons other than suspected adrenal pathology.
However, once discovered, two main questions arise: firstly, is the incidentaloma associated with hypersecretory syndrome and second, is the lesion benign or malignant.
It is well-recognized that many incidentaloma, previously considered to be non-functional, display subtle states of cortisol excess, dubbed "pre-Cushing's syndrome" or "subclinical Cushing's syndrome" (biochemical evidence of Cushing's syndrome without physical stigmata).
