Incidents in Aviation in the Aviation History Encyclopedia
The Tenerife disaster which killed almost 600 people is often regarded as the worst-ever incident in Aviation history.
The "worst ever aviation incident", in other words, is a matter of definition.
Two incidents of note have happened in Puerto Rico: On December 31, 1972, baseball player Roberto Clemente, his helpers and crew died when the DC-3 they rented for a relief flight to Nicaragua crashed off the Atlantic coast, shortly after take-off from Luis Munoz Marin International Airport, then known as Isla Verde International Airport.

Hijacking and commandeering incidents are viewed within the context of the U.S. Federal criminal statute (49 USC 1472 (i)), which defines air piracy as any seizure or exercise of control, by force or violence or threat of force or violence, or by any other form of intimidation, and with wrongful intent, of any aircraft.
The incident began shortly after takeoff of flight MU 5915 when the man announced that he was carrying a bomb concealed in his bag and demanded to be flown to Taiwan.
In this incident, a passenger had placed the device for an apparent suicide attempt; however, he was not the individual killed in the blast, which tore a hole in the plane’s fuselage.

An aviation incident is defined as an occurrence other than an accident, associated with the operation of an aircraft, which affects or could affect the safety of operations.
Other countries adopt a similar approach, although there are minor variations, such as to the extent of aviation-related operations on the ground covered, as well as with respect to the thresholds beyond which an injury is considered serious or the damage — substantial.

According to Dr Hussain Pervaiz, a Dubai-based aviation safety and medical expert who trained with Nasa, an increase in deceptive advertising techniques, general poor service and unprofessional cabin crew are also contributing to air rage.
He said airlines are not admitting publicly to the rising incidents of aviation rage because of market repercussions, and claims only the results rather than the causes are being treated.

Undercurrent: Dive Gear and Dive Deaths; Equipment Problems? or Diver Error?; February, 2000
DAN could contribute more to diver safety if it would take the tack of the Australians, who recently analyzed 1000 "incidents," finding that 105 (more than 10 percent) were "pure" equipment failure, a percentage similar to incidents in aviation, medicine, and industry, where studies show that 8-10% of the incidents arise from equipment failure.
In two incidents, an alternate air source (a power inflator and demand valve combination) developed a leak during the dive (a pre-dive check would not have detected this fault), requiring disconnection to preserve the diver's air supply.
In ten incidents, the inflator spontaneously inflated the BC: in seven of these cases, the diver failed to slow the rapid ascent and died.
