In mathematics, if A is a subset of B, then the inclusion map (also inclusion function, or canonical injection) is the function i that sends each element of A to the same element in B:
Inclusion maps tend to be homomorphisms of algebraic structures; more precisely, given a sub-structure closed under some operations, the inclusion map will be a homomorphism for tautological reasons, given the very definition by restriction of what one checks.
Inclusion maps in geometry come in different kinds: for example embeddings of submanifolds.

Inclusion can function on many levels, where the disabled student joins the general education classroom for part of the day, depending on the type and severity of the disability, to full inclusion where all students are in the general education classroom instead of the special education classroom.
Full inclusion should allow disabled students to be part of the heterogeneous surroundings, teach social skills, foster independence and provide opportunities to build friendships with non-disabled peers (Fuchs and Fuchs as cited in Petch-Hogan, Haggard, 1999).

 Strange Crash On Inclusion of Function - GameDev.Net Discussion Forums
I tried walking up the stack and I found nothing to do with toolbar_t, it's crashing after calling some STL functions from the copy constructor of toolbarIcon_t, which is used in the code before toolbar_t ever is. I can't find any reference to toolbar_t anywhere.
I vaguelly recollect having a problem similar to this happening a month ago and it was "fixed" by putting the function into a different source file.
It's much more probable that you have a more subtle bug in your code and moving the function to a different file is causing sufficient change in your programs layout in memory to mask the problem.
