Gallium imaging - Wikipedia, the free encyclopedia
Gallium Imaging is a nuclear medicine method for the detection of infections and cancers.
Radioactive gallium-67 binds to transferrin, leukocyte lactoferrin, bacterial siderophores, and inflammatory proteins.
Gallium imaging is becoming an obsolete technique, with indium leukocyte imaging and technetium antigranulocyte antibodies replacing it as a detection mechanism for infections.

 Indium
Indium phosphide was nominated for study because of its widespread use in the microelectronics industry, the potential for worker exposure, and the absence of chronic toxicity data.
Exposure to indium phosphide also caused inflammation of the arteries of the heart, primarily the coronary arteries and the proximal aorta, and to a lesser extent the lung-associated lymph nodes in males and in females.
Exposure to indium phosphide by inhalation resulted in nonneoplastic lesions in the lung of male and female rats and mice, the adrenal medulla of female rats, and the liver and heart of male and female mice.

Distinguishing osteomyelitis from Charcot neuropathy is difficult and imaging is affected by the stage and degree of activity of the Charcot osteoarthropathy process (6).
Imaging is performed at 4h, 24h and, in cases of chronic osteomyelitis, at 48h.
Sella et al have further stated that combined leukocyte and marrow imaging is the current gold standard for evaluating the presence of diabetic foot infection vs. osteoarthropathy.

Indium oxime cannot be used to label the cells if they are still suspended in plasma as 90% of the tracer binds to transferrin.
Unfortunately, gallium imaging is hindered by the same lack of specificity as conventional bone scintigraphy as gallium localizes to bone as a calcium analog and it is incorporated into the calcium hydroxyappatite crystal.
Pulmonary uptake of leukocytes is presumably due to non-specific inflammatory responses that stimulate leukocyte accumulation in the lungs through increased vascular permeability due to complement activation, endothelial damage, or pulmonary vascular bed vasodilatation.

A Closer Look At Nuclear Medical Imaging
Combination imaging to differentiate soft tissue infection from osteomyelitis has been repeatedly suggested in the literature to enhance the specificity and sensitivity of nuclear medicine leukocyte studies.
To interpret radiolabeled leukocyte studies appropriately, it is important to understand disease states other than infection that prompt leukocyte accumulation in order to predict when a false positive image may result.
Understanding this phenomenon is integral to an accurate interpretation of leukocyte imaging in clinical conditions that involve Charcot neuroarthropathy and the risk of coincident infection.

Nuclear Medicine Imaging of the Gastrointestinal System - IBD
Imaging with In-111 or Tc-99m labelled leukocytes offers a number of significant advantages over gallium (Ga-67) citrate and is the procedure of choice for detecting inflammatory bowel disease.
Imaging is generally performed 48 hours post-injection but earlier images obtained at about 6 and/or 24 hours may enhance the diagnostic quality of the procedure.
Imaging times may be as long as 10 to 15 minutes depending upon the activity administered and the total counts acquired for the image.

Summary of Work: The kinetic patterns of fresh, frozen-thawed, or cultured human leukocytes are studied by tagging the cells ex vivo with 111-Indium, a radioisotopic label, and measuring their distribution throughout the body by means of gamma camera imaging and gamma counting of blood samples.
Three of the 23 Indium-labeled allogeneic leukocyte studies were performed in patients with chronic granulomatous disease, in one case to document clearance of pulmonary infection, in the second to confirm lack of trafficking in an alloimmunized recipient, and in the third to localize a cause for fever.
In all three cases, gamma imaging results were used to guide therapy, specifically to terminate a course of granulocyte transfusions when they were either no longer necessary or were ineffective or to prevent an exploratory laparotomy.
