Induction (birth) - Wikipedia, the free encyclopedia
Induction is a way of artificially bringing on labour in a woman.
Inductions should only be undertaken for significant medical reasons.
If induction does not work, a Caesarean section may be required.

VBAC is Safe!   (Site not responding. Last check: 2007-10-28)
Induction of labor, epidural anesthesia, failure to progress, and abnormal fetal heart rate pattern are all associated with failure of a trial of labor and uterine rupture.
Induction or augmentation of labor with oxytocics was associated with 50% of complete ruptures and 25% of dehiscence.
Although a vaginal birth after a cesarean is considered safe in modern obstetrics, it is not known whether repeated VBACs increase the risk of rupture, or whether the first VBAC proves the strength and durability of the scar, predicting further successful and less risky vaginal deliveries.

Induction of Labor - California Pacific Medical Center's Women & Infants Center
Often, an induction of labor is performed to manage selected medical complications of pregnancy (including a birth that is significantly overdue) and, in certain situations, for the elective timing of delivery.
Inductions are an important consideration for a number of medical conditions, even in a first birth.
Induction may be useful if you live a distance from the hospital or in other circumstances to optimize your care.

Giving birth: Is induction for convenience safe?
Induction of labor, by whatever means, holds fundamental risks to mother and child and comes at a higher economic cost as well.
Invalid reasons for induction include: discomfort in the last few days of pregnancy, a normal pregnancy that goes beyond the due date, a doctor who is going out of town, a planned visit by a family member, new jobs, trips and tax relief.
Although the literature is divided on whether or not induction of labor leads to a higher incidence of cesarean birth, it is an intervention which tends to medicalize the birth process and takes control away from the mother and father.

Although induction of labor is recommended as a therapeutic option only when the benefits of expeditious birth outweigh the risks of continuing the pregnancy, a "psychosocial indication" has become a common rationale for elective induction in the United States.

VBAC and Pharmaceutical Induction: Help or Hindrance?
Induction is medically indicated only if you or your baby have a specific health problem, such as diabetes mellitus or eclampsia.
Although birth is certainly possible when the baby's head is asynclitic (tilted or turned toward one side instead of tucked), asyncliticism is another obstacle which it is preferable to avoid.
The induction adds to the stress the baby experiences, increasing the risk of fetal distress, especially where higher doses of oxytocin or misoprostol are used.(5)(6)(7) You must be carefully monitored because induction can lead to overly strong contractions.

Elisa's spouse, Travis Casey, is to be present throughout the entire labor, birth, and recovery period, including all procedures and examinations unless she specifically asks him to leave.
Elisa prefers that induction is initiated with prostaglanden gel if her cervix is not ripe and that pitocin is used as a last resort and in minimal amounts required to obtain the desired results.
At the same time I recognize that there may be times that guidance is needed and that the midwife or OB who is attending the birth should be the one doing the directing as, well, they have the bird's eye view of what is happening.

Induction Dangers   (Site not responding. Last check: 2007-10-28)
Following are a small sample of studies and research showing that inductions lead to cesareans, instrumental delivery, shoulder dystocia (from the pelvis not being allowed to naturally relax and open to allow the proper passage of the baby), death, and more.
One of my concerns about home inductions is that the birth which is stimulated by outside forces can result in erratic birth processes that stop and start, and are difficult to complete.
Among all women induction was associated with modest increases in instrumental delivery (19% vs 15%; relative risk, 1.20; 95% confidence interval, 1.09-1.32) and shoulder dystocia (3.0% vs 1.7%; relative risk, 1.32; 95% confidence interval, 1.02-1.69).

GD Birth Stories   (Site not responding. Last check: 2007-10-28)
Induction probably succeeded so early because she was already partially effaced and dilated before they began, although they did nothing to ripen the cervix ahead of time.
Also note that Laura had a doula for her induction; although research on doulas during inductions is limited, we know that they help cut the c/s rate by about 50% in non-induced labors, and one small study seems to indicate that they also cut the c/s rate in inductions very significantly as well.
She had a midwife and doula for her birth, both of which tends to cut the c/s rate significantly, and she was able to use water for laboring as well as a standing position for pushing and birth.

Results: Among women with spontaneous labor, women with first birth cesarean delivery had a 30% increase in risk of abruption at their second birth relative to women with vaginal delivery (95% CI, 1.1-1.6).
The rate of uterine rupture among women who underwent second birth induction of labor was higher (9/1000 cesarean procedures) than among women with cesarean delivery who labored but were not induced (5/1000 cesarean procedures).
Conclusions: Women with first birth cesarean were at increased risk for adverse utero-placental outcomes at second birth, and induction status altered the risks of abruption and uterine rupture.

OB/GYN News: High birth weight, induction boosted shoulder dystocia risk
The standout statistic, however, was birth weight, with 19.3% of shoulder dystocia cases associated with a birth weight of more than 4,500 g, compared with 1.5% of non-shoulder dystocia cases.
Limitations in estimating accurate birth weight antenatally, however, prevent its use as a practical means of predicting shoulder dystocia, he added.
While the clinical triad of oxytocin use, labor induction, and birth weight of more than 4,500 g yielded an odds ratios of 23.2 for shoulder dystocia, its sensitivity and positive predictive value were only 12.4% and 3.4%, respectively.

I find there is a fine line between natural and synthetic ways for induction of labor, so I will do an overview which will cover both ways.
First of all, any of the ways I describe to induce labor should never be attempted without the full knowledge of your healthcare provider, and these are only to be done when you are full-term.
