Receiving an intentional base on balls does not count as an official at bat for a batter but does count as a plate appearance, and a base on balls.
The Marlins' Miguel Cabrera hit an intentional ball thrown by Todd Williams during the 10th inning resulting in a run scored for the Marlins.
The purpose of an intentional walk is to bypass a good hitter in order to face a batter that the defensive team feels they have a better chance at getting out, or to set up a double play ball by putting a runner on first base.

The original Cartwright Rules stated: "Three balls being struck at and missed and the last one caught is a hand out; if not caught, is considered fair, the striker bound to run." A batter could take all the pitches he wanted until he got one he wanted to hit.
This did nothing to prevent the pitcher from throwing wide, though, and in 1863, the "ball" was invented, and batters awarded first base when, in the judgment of the umpire, and after a warning, the pitcher had thrown three unfair balls before the batter had struck out.
The intentional walk has it's utility, there are situations where it's a good idea to use it, and it shouldn't necessarily be eliminated because it's being exploited.

I agree that the intentional walk is a blight on the game, and in fact is an unfair practice.
Intentional walks are a legitimate strategy and have been part of the game forever.
I've also seen a game end where a pitcher tried to throw an intentional ball and ended up throwing it right down the middle and the hitter blasted it off the left field wall.

When play stops and the ball becomes dead, a runner who occupies a base beyond the one he/she was entitled to steal, the runner will be returned to the correct base without liability to be put out.

I was wondering on scenario 2 that the ball which is caught in midair could be interpreted as a pass to oneself which is illegal (btw do you know the rule reference for that).
When the ball leaves the grasp of a players hands and he touches it before another player touches it, it is only one of three things: dribble, shot, or fumble.

A qb could theoretically drop back and throw the ball at his feet if he was not being pressured by the defense and it would be ruled an incomplete pass (after some argument and discussion among the coaches and officials I'm sure).
This is why on the botched Giants playoff FG a few years back the holder couldn't have thrown the ball into the ground to stop the clock and go to 4th down.

Leonard Koppett suggests that balls hit be scored separately from strikes, but I think that goes too far.

As I acknowledged, the ball is live during an IBB, and folks seem to relish the booing and the rare swing/pickoff andc.
But if a pitcher tried to intentionally walk a batter, the batter could simply decline until either the batter scored a hit, the batter was put out, or (very unlikely) the batter received a total of 16 walks and thus a free home run.
