According to fluoroscopy the transseptal needle had a correct position, but the intracardiac echo image showed that it was actually pointing towards the aortic root and most importantly, that it was virtually impossible to stabilize it in the fossa itself.
Based on intracardiac echo findings a decision was made to limit the procedure only to ablation of the cavotricuspid isthmus and not to proceed further so as to avoid complications.
This case report illustrates the usefulness of the intracardiac echocardiography in preventing serious or even fatal complications in transseptal procedures when the cardiac anatomy is unusual or distorted.

In two studies of TTE among young patients (aged 15 to 45) with stroke, the pooled prevalence of intracardiac thrombus in 180 patients was 2.2 percent (similar to that in unselected patients).
The overall yield of intracardiac thrombus using TEE in consecutive stroke patients was 1.7 percent (95 percent CI, 0.5 to 5.3 percent).
Because of the relatively low prevalence of intracardiac thrombus in patients with stroke, it is difficult to assess the accuracy of TTE and TEE in this population.

After a dive which allows venous bubble formation, a diver who has an intracardiac right-to-left shunt is at risk of paradoxical gas embolism, which can result in neurological decompression illness(1,2).
It therefore seems reasonable that sport divers known to have an intracardiac shunt should be allowed to dive shallower than 15 metres, provided no other cardiac contraindication exists.
If a diver with a shunt wishes to go deeper than 15 metres the options include use of nitrox with an air decompression table (to reduce bubble liberation and tissue nitrogen-load) and use of a table such as the DCIEM table which is believed to result in little or no bubble nucleation.

Transposition of the great vessels (TGV) is a form of congenital heart disease, in which the aorta arises from the right ventricle and the pulmonary artery arises from the left ventricle, with an associated intracardiac shunt.
As a safety mechanism, an intracardiac shunt (ie: an atrial septal defect) opens, and this allows the oxygen-rich blood that returns from the lungs to partially mix with the blood in the right ventricle, allowing oxygen to be delivered to the organs of the body.
While the x-ray may be normal at birth, the cardiac sillouette gradually enlarges into a characteristic globular or egg-on-its-side appearance.

In this study decision analysis was used to assess the risks and benefits of three courses of action: (1) primary intracardiac repair; (2) palliative operation in infancy and delayed intracardiac repair; and (3) treatment with propanolol in infancy and delayed intracardiac repair.
The choice of action was affected most by the mortality of primary intracardiac repair and the likelihood of a good hemodynamic result from intracardiac repair.
It was found that in the institution studied if the mortality rate of primary intracardiac repair is 10 percent or less, intracardiac repair should be performed; otherwise propanolol treatment, which allows delay in intracardiac repair, should be selected.
