Crocheted fabric in the modern sense is begun by placing a slip-knot loop on the hook, pulling another loop through the first loop, and so on to create a chain.
An exception is afghan crochet where at the beginning of row one, (and all odd numbered rows) there is one loop on the hook, and one works across the row picking up loops and then in the second row(even numbered rows) the loops are successively worked off.
Crocheted laces in the new Edwardian era, peaking between 1910 and 1920, became even more elaborate in texture and complicated stitching.

Pinch where the thread crosses over itself, and just crochet chains using the thread on top of your hand, which is the thread that emerges from the last stitch.
The loop of thread around your hand gets smaller as you crochet, but since picots don't have many chains, that's OK. When you have one more chain than the pattern calls for, remove your hand from the loop and pull it firmly shut before you go on to the chains after the picot.
