The city of Putchezh, located in the Ivanovo Oblast to the southeast of the town of Myshkin, is one with a unique and curious history.
Nearby is Ivanovo City, capital of the Oblast, which became knows as the "fiancée's city": A large number of female workers would immigrate to the town to work in the weaving industry (which fed off of Putchezh's flax trade), and legend has it that single men would come here in search of wives.
It is bordered by five other Oblasts: Ivanovo to the north, Ninhnii Novgorod to the east, Ryazan to the south, the Moscow Oblast to the west, and Yaroslavl on the northwest corner.

In Orel and Ivanovo, there was no strong evidence that any of the risk factors examined were associated with cure rates — for example, in Ivanovo oblast low cure rates were observed even in patients with no history of alcohol abuse.
This could initially be done using individual oblasts as the unit of analysis, followed by extrapolation to the national level; Treatment appears to be provided in a similar way across oblasts, despite differences in cure rates, financing mechanisms, geography and income levels.
The cure rate in Ivanovo City also significantly improved; more evidence on costs and effectiveness are required, because the existing evidence from Ivanovo is insufficient to rule in or rule out such programmes; and if existing projects are to be sustained and/or expanded to many other parts of the country, new sources of funding (e.g.

Primary Multidrug-Resistant Tuberculosis -- Ivanovo Oblast, Russia, 1999
Despite implementation of DOTS in Ivanovo in October 1995, as of October 1998, approximately 30% of 514 never-treated (defined as less than 1 month of previous treatment), smear-positive TB patients had poor treatment outcomes (i.e., relapse, treatment failure, or death).
However, in communities such as Ivanovo that have high levels of drug resistance, additional strategies are needed for patients with drug-resistant TB, including rapid assessment of drug susceptibilities and use of alternative, "second-line" TB drugs (5,6).
Drug resistance contributed to 17% of the poor treatment outcomes observed in Ivanovo; however, most of these poor outcomes probably were associated with delayed diagnosis, treatment interruption, failure to ensure patient adherence, and other program-related factors (5).

The Centers for Disease Control and Prevention, WHO, and U.S. Agency for International Development are helping implement basic DOTS programs in the Orel and Vladimir oblasts (regions) and strengthen the DOTS program started in 1995 by WHO in Ivanovo oblast.
Treatment success -- defined as a patient who completed treatment or who's cure can be documented using bacteriological tests -- was achieved in 88 percent of the new civilian TB cases and 60 percent of the civilian patients being re-treated for the disease.
CDC suggests that this success may be due to the dedication of TB control staff to ensure patients complete treatment, lower rates of multi-drug resistant TB, and the possibility that patients are seeking treatment sooner rather than later.
