John Q is a 2002 movie starring Denzel Washington as John Quincy Archibald, a father and husband whose son is diagnosed with an enlarged heart and then finds out he cannot receive a transplant because insurance won't cover it.
John is a factory worker in a mining town in Illinois.
John's gunshot wound is treated by the emergency room staff, and using the bound SWAT policeman as a human shield, steps outside to the sight of dozens of policemen pointing weapons at him, as well as a large crowd gathered to support his cause.

As for John's hostages, they consist of the necessary random mix of people, none of which are interesting.

Afterwards he introduces us to John Q. Archibald (Washington), a struggling blue collar guy and stand-up father whose factory job has been cut down to 20 hours a week, thus disqualifying him from full insurance coverage.
Meanwhile, in the hospital's ICU, John's wife and son, and all the doctors and nurses, are somehow unaware that hostages have been taken in another part of the building until a sympathetic police negotiator (Robert Duvall) comes to ask the wife to talk John down.
If the rest of "John Q" was half as sincere and self-possessed as this brief moment, it might have been a brilliant and moving film.

And on a social level, John Q succeeds in making a powerful comment on a subject that demands a lot more attention than it has been given.

Unfortunately, John Q doesn't presume that you see this obvious answer, and goes on to instruct you as to the evil of this system, with several dialogue explications and Arnoldish cracks ("The hospital's under new management now!") sprinkled amidst its one-note, ongoing crisis of a plot.
As John looks on in horror, Payne and Turner announce that a heart transplant is Mikey's only chance to live, but that he and Denise have no insurance and so, should just settle for a few more "quality of life" weeks with their boy.
And still, after all this excess and strangeness, John Q absolutely outdoes itself in the closing moments, when, under the guise of promoting its populist hero, it comes up with what has to be the creepiest movie moment in recent memory.

Duvall's detective role is flat and you learn nothing by the end of the film about him or just the mere ability to be glad at the end of the film to root for him and his accomplishments throughout.
John Q, is a prime example of how important it is to have some character development for your supporting roles and that no matter who gets the title role, the actor or actress will have a hard time in making it work.
