 Scientific medical journals like JAMA fail basic credibility standards; medical journals become increasingly irrelevant
The Journal of the American Medical Association -- JAMA -- and other scientific medical journals have been caught red-handed by the Center for Science in the Public Interest for failing to disclose the financial relationships between study authors and companies that might benefit from such studies.
A number of journals were caught in the same study, revealing that this failure to disclose conflicts of interest is not merely something that happens at the American Medical Association, but something that is widespread in the conventional medical community.
Too many of these journals are masquerading as stewards of good science -- they pretend to show articles that are well-researched, that are authored by people who have no financial interest in their publication, and that have been put through a rigorous quality control process known as peer review.

Published in the November 27, 2002 issue of the Journal of the American Medical Association (JAMA), the study suggests that physicians treating patients in acute kidney failure should reassess the use of diuretics, particularly when there is a limited response in terms of increased urine output.
The 552 patients studied had been admitted for a variety of medical conditions, and were then diagnosed with acute renal (kidney) failure in intensive care units at UCSD Medical Center, the San Diego Veterans Affairs Medical Center, San Diego Naval Hospital, and UC Irvine Medical Center from October 1989 to September 1995.
An accompanying editorial in JAMA said the study is timely and clinically important because administration of diuretics to ICU patients in kidney failure is still a relatively common practice.

Levine and colleagues studied the correlates of neutropenia in the HIV-infected women enrolled in the Women's Interagency HIV Study and found that development of neutropenia was associated with worsening HIV disease parameters, such as increasing HIV viral load and decreasing CD4 cell count.
Improvements in HIV disease parameters were associated with resolution of neutropenia, as was the use of highly active antiretroviral therapy.

In a prospective longitudinal study of depression during pregnancy, Cohen and colleagues assessed risk of depression relapse in women who discontinued antidepressant medication compared with women who continued antidepressant use.
They found that 43% of the women experienced a relapse of depression during pregnancy; in addition, 26% of women who maintained their medication throughout pregnancy relapsed compared with 68% who discontinued medication.
JAMA and the Archives Journals will publish a joint theme issue devoted to pain management in November 2003.
