On fourth voyage 1499 Cosa was the first pilot for the expedition of Alonso de Ojeda and Amerigo Vespucci, and was with them amongst the first to set foot on the South American Continent on the Gulf of Paria.
Cosa was nominated alguazil major, and in 1504-05(?) (or 1506) was commander of an expedition to the Pearl Islands and the Gulf of Uraba to found settlements there.
Juan de la Cosa made several charts of which one, Mappa Mundi (1500), the famous chart of the world is still preserved.

De la Cosa drew the map in 1500 (some say 1502) and signed it beneath the image of Saint Christopher on the west end of the map.
For example, four English banners in the north, and the inscription Mar descubierta por los Ingleses, recall the voyages of John Cabot (1496-1498); an inscription on the Coast of Brazil states, "this cape was discovered in the year 1499 by Vincenti," meaning Vincenti Yañez Pinzón, whose expedition landed there.

After accompanying Alonso de Ojeda in 1499, he drew (1500) a world map (a manuscript copy exists in Madrid) that seems to be the first to question the identification with Asia of the new lands and to furnish evidence that the Cabots coasted farther S along the Atlantic shore than other documents reveal.
In 1501 he was with Bastidas, and later (1504) he again explored the northern coast of South America.

In 1510 Diego de Nicuesa, another Spanish explorer, established the settlement of Nombre de Dios at the mouth of the Chagres River, and to the southwest, Alonso de Ojeda founded San Sebastian de Urabá.
With the final destruction of Nombre de Dios in the late 16th century by the Englishman Francis Drake, commercial activity was moved to the hamlet of Portobelo, overlooking the calm bay recorded by Christopher Columbus in 1502.
