I believe, however, that if we properly explain to our patients the devastating consequences of a hip fracture and the fact that these pads can be extremely effective in reducing their occurrence, most patients will readily accept them.
Kannus and his colleagues in Finland reported the results of the use of an external hip protector in elderly people who were identified at significant risk for hip fracture.
The data provided by Kannus and colleagues provide us with another effective method of prevention in addition to the pharmacologic approach and those falls-prevention measures currently being utilized.

An estimated 250,000 people break their hips every year in the United States, and the National Institutes of Health predict that an aging population will push that to 650,000 by midcentury.

Of the 13 patients in the study that broke their hips after being assigned a hip shield, nine were not wearing them at the time of the incident.
Kannus suggested that hip shields were analogous to bicycle helmets.

In male twins the familial resemblance is explained by a significant contribution from genetic factors but not by the shared family environment, with genetic factors accounting for 35% of the variation in liability to fracture.
A greater genetic contribution is also suggested at the spine in table A in Kannus et al's study (bmj.com/cgi/content/ full/319/7221/1334/DC1), although inference is limited by the small numbers of concordant pairs and the lack of data on differences between the sexes in rate of fracture.
In contrast to the conclusion reached by the authors, these data show that genetic factors contribute to a third of the liability to osteoporotic fracture in men and are entirely compatible with the hypothesis that genetic factors contribute to a similar extent in women.

Kannus' editorial promotes physical activity for the prevention of injurious falls among elderly people.
He acknowledges the uncertainty that surrounds the effectiveness of specific exercise programmes tested as interventions in randomised controlled trials, in contrast to the epidemiological evidence from longitudinal cohort or case control studies.
In the meanwhile, we whole-heartedly agree with Kannus that regular physical activity outside of formal exercise programmes is likely to be beneficial to younger and older people.
