It is an expansion board for the original Save Doctor Lucky, which was itself a sequel (of sorts) to Kill Doctor Lucky (which had an expansion board of its own, based on an actual mansion).
You could only make an attempt on the Doctor if you were in the same room with him, and no other player could "see" you commit the crime.
Like many Cheapass games, Kill Doctor Lucky is a nice way to fill time on a Friday night if half your regular game group doesn't show up, or the movie you rented is horridly awful, or you're looking to entertain some relatives who are visiting for the weekend.

If Dr. Lucky stumbles upon a corpse his skill rises by one point per corpse, as does his attack which is equal to his resistance.
If one Lucky pawn lands in a room with the other Lucky pawn, they move again, even if someone was in the room (that person doesn't get an extra turn as he/she is too weirded out by seeing Lucky in two places at once).
If Doctor Lucky makes it to this room, and is by himself, he flips the switch and a clone is produced.

Killing the doctor is a deceptively difficult task.
However, because of the way the board is constructed and because Doctor Lucky doesn’t count hallways and stairways in his movement (and players do), a player cannot perform this trick indefinitely.
While Doctor Lucky moves in a predictable path, this rule (and the fact that players can play cards to move Doctor Lucky more rooms or even instantly transport him to a given room) means that setting up an ambush can be quite difficult, even frustrating.

Kill Doctor Lucky meanwhile is one of those games that improves with more players.
Dr Lucky is a daft old buffer, clearly a bit deranged, and therefore moves on a programmed route, every turn, around the thirty odd rooms visiting his guests as he goes.
Even better is to have a couple of weapons so you can try to kill him on one turn, draw out most of the failure cards, and then get him next turn with something else before the others have had the chance to replenish.

The premise of KDL is deliciously evil and turns the game of Clue on its head: this is a pre-mystery game in which the object is not to discover who committed the murder, but rather to succeed in committing the murder (I did warn you that you might not consider this appropriate for the kids).
Dr. Lucky travels through the house according to his own rules: each room is numbered and at the end of each turn, Dr. Lucky proceeds to the next sequential room (whether connected by a door or not).
Once you attack Dr. Lucky (if you happen not to have a weapon in hand, fear not: you can always 'poke him in the eye' for a single point of damage), the other players may play special cards to deflect the damage and save Dr. Lucky.

Back in the Nineties, having seen the 3D boards Games Workshop built to demonstrate Space Hulk at their conventions, I started to plan my own and even got as far as building a couple of "mock ups" of small sections but no further.
To help choose from the hundreds of things they sell, they list a few of their favourites, which includes Kill Doctor Lucky by Cheapass Games.
If you compare what I've done with the standard board you'll notice that there are fewer numbers (which are used during the game to determine how Dr Lucky moves around his mansion) and more arrows.
