  LVEF
LVEF is a good clinical indicator of left ventricular systolic function.
LVEF may be determined either by echocardiograph or radionuclide ventriculography.
LVEF is the fraction of blood ejected in systole and is calculated as follows:

Cardiac monitoring essential for preventing heart disease in patients with blood disorder   (Site not responding. Last check: 2007-11-04)
A team of researchers from University College London Hospitals studied 81 thalassemia major patients who had no previous cardiac history and whose LVEF readings were monitored on a yearly basis for a median of six years to a maximum of 20 years, depending on the patient.
It was determined that if an LVEF reading was less than 45 percent (indicating a weakening of the heart muscle), or if the reading fell 10 percent between consecutive measurements, the patient's risk of developing symptomatic heart disease was 25 times greater than in patients with normal readings.
The study found that the median period of time between an abnormal LVEF reading and development of cardiac disease was three and a half years — enough time for treatment with DFO to prevent and reverse the severe heart dysfunction.

Selection and Treatment of Candidates for Heart Transplantation
Improvement of LVEF by more than 5% was a very favorable prognostic sign and identified a subgroup of patients with a mortality risk of less than 10% over a 1-year period.
Among 466 patients with LVEF less than 40% and angina, 36% of whom had clinical heart failure, survival with surgical revascularization was significantly greater (P=.001) in patients with LVEF greater than 20% than in those with LVEF less than 20%, both in hospital (89% versus 63%, respectively) and 3 years postoperatively (60% versus 15%, respectively).
In comparison with patients with a normal LVEF, a larger number of patients with LVEF less than 50% have inducible ventricular tachycardia, which is not necessarily a specific predictor of the subsequent risk of arrhythmogenic death.

No person with a baseline LVEF that was less than 50% was considered a candidate for treatment with Novantrone.

Cardiac toxicity: A patient's baseline left ventricular ejection fraction (LVEF) should be evaluated by echo contrast or a multiple gated acquisition (MUGA) scan and should be >50%.

LVEF should be reevaluated by echocardiogram or MUGA prior to each dose administered to patients with multiple sclerosis.

The exact role of this assay is still being defined, but the assay will probably be most useful when heart failure is suspected but remains unconfirmed after the medical history has been carefully obtained and physical examination, electrocardiography, and radiography of the chest have been done.
For patients with LVEF less than 35% and severe symptoms (such as fatigue or dyspnea with minimal activity), spironolactone (an aldosterone blocker) also decreased mortality.
Survival data for patients with heart failure and normal LVEF are lacking, and treatment for these patients is empirical but ultimately includes many of the same medications used to treat systolic heart failure.

An implantable cardioverter-defibrillator is recommended as secondary prevention to prolong survival in patients with current or prior symptoms of HF and reduced LVEF who have a history of cardiac arrest, ventricular fibrillation, or hemodynamically destabilizing ventricular tachycardia.
Placement of an implantable cardioverter-defibrillator is reasonable in patients with LVEF of 30% to 35% of any origin with NYHA functional class II or III symptoms who are taking chronic optimal medical therapy and who have reasonable expectation of survival with good functional status of more than 1 year.
Patients with LVEF less than or equal to 35%, sinus rhythm, and NYHA functional class III or ambulatory class IV symptoms despite recommended, optimal medical therapy and who have cardiac dyssynchrony, which is currently defined as a QRS duration greater than 120 ms, should receive cardiac resynchronization therapy unless contraindicated.

T-wave alternans could surpass LVEF as risk-stratifier for ICD implantation
In the T-wave Alternans in Heart Failure (TWA in HF) trial, the TWA test "was at least twice as strong in predictive value as ejection fraction, which is an incredibly good risk stratifier," Dr J Thomas Bigger (Columbia University, New York, NY), told heartwire.
The population consisted of 549 mostly male patients in sinus rhythm and an LVEF <40% (average 25%) who were in NYHA class 1-3, divided nearly evenly between those with ischemic and nonischemic cardiomyopathy.
Although at least one prior study has suggested that TWA isn't predictive of death and ventricular arrhythmias among patients with a prolonged QRS interval,[5] according to the common definition also used in Bigger's study, retrospective data from the TWA in HF trial suggests the opposite.
