  Uterine Leiomyosarcoma
Uterine leiomyosarcoma (LMS) is a rare form of cancer of the uterus, where smooth muscle tumors develop in the lining of the uterus.
Uterine leiomyosarcoma is sometimes confused with the most common type of uterine cancer, called endometrial adenocarcinoma, because both cancers originate within the uterus.
Leiomyosarcoma occurs in the smooth muscle of the uterus called the myometrium, while endometrial adenocarcinoma develops in the lining of the uterus (the endometrium).

A leiomyosarcoma is one of a family of cancers called soft tissue sarcomas.
Leiomyosarcomas are so-called because they arise from cells very similar to those found in a type of muscle tissue called smooth muscle.
Leiomyosarcomas usually start as a fairly rapidly growing, painless, swellings in the wall of an organ or a blood vessel.

Pathology and Pathophysiology of Uterine Smooth-Muscle Tumors
Intravenous leiomyomatosis is distinguished from leiomyosarcoma by its lack of mitotic activity, atypia, and coagulative necrosis, and from endometrial stromal sarcoma by the demonstration that it is composed of smooth muscle.
The leiomyosarcoma, a malignant tumor composed entirely of smooth muscle, is the malignant counterpart of the leiomyoma and is the most common pure sarcoma of the uterus.
Most leiomyosarcomas irregularly invade the adjacent myometrium and have a cut surface that is pale with areas of hemorrhage and necrosis.

 Leiomyosarcoma - Children's Hospital Boston
Leiomyosarcoma accounts for 7 to 11 percent of all soft tissue sarcomas among adults and children.
The exact cause of leiomyosarcoma is not entirely understood, however, studies have indicated that genetic alterations may play a role in the formation of all soft tissue sarcomas.
A leiomyosarcoma may be localized, meaning it has not spread beyond the muscle tissue where it arose or beyond nearby tissues, or metastatic, meaning it has spread to lungs, bones or to other organs or structures of the body.

Leiomyosarcomas on the arms and legs may be treated by amputation (removal of the affected limb) or by limb-sparing surgery to remove the tumor.

Leiomyosarcoma is a soft tissue sarcoma that attacks smooth muscle cells, usually in the gastrointestinal tract, retroperitoneum, and uterus.
The size and grade of leiomyosarcoma tumors are the primary determinants of prognosis, which is typically poor since the cancer is very difficult to detect until it is in an advanced state and begins to produce symptoms.

Our doctors have developed a novel chemotherapy regimen that has demonstrated excellent efficacy in patients with leiomyosarcoma -- a sarcoma that arises within the uterine wall -- that has metastasized and cannot be removed surgically.
The success of this regimen in leiomyosarcomas has led to additional research efforts, including a larger study of our treatment regimen by the Gynecologic Oncology Group.
For patients with early-stage leiomyosarcoma of the uterus that has been completely removed surgically, we are evaluating chemotherapy based on the regimen that has shown high response rates in patients with metastatic disease, given in sequential fashion with radiation therapy and additional chemotherapy.
