  Data Sheet
Lignocaine is contraindicated in patients with a known hypersensitivity to local anaesthetics of the amide type.
Lignocaine should be used with caution in patients with epilepsy, liver disease, renal disease, congestive cardiac failure, marked hypoxia, severe respiratory depression, hypovolaemia or shock and in patients with any form of heart block or sinus bradycardia.
It may be necessary to reduce the dose of lignocaine in elderly or debilitated patients, acutely ill patients and children commensurate with their age and physical status.

Systemic local anaesthetic-type drugs in chronic pain
Intravenous lignocaine is effective in fibromyalgia (based on small patient numbers), but not in cancer related pain.
Lignocaine and related local anaesthetic-type drugs which block sodium channels have been used to relieve clinical pain, as a last resort in cancer-related pain and in other conditions when more traditional treatments fail.
In three of three trials lignocaine was no different to placebo (conditions: bony metastases, chemotherapy polyneuropathy, radiotherapy plexopathy, tumour invasion of nerve plexus).

 Lignocaine for acute migraine
Lignocaine and other local anaesthetic-type drugs which block sodium channels have been used to relieve chronic clinical pain.
Lignocaine is an amide type local anaesthetic, and has been studied in the treatment of acute migraine attacks.
The second trial of intravenous lignocaine (up to 2.7 mg/kg) was not as effective as intravenous chlorpromazine, but as effective as intravenous dihydroergotamine.

Prospective study of the adverse effects of IV lignocaine in 106 patients with fibromyalgia; retrospective questionnaire study of the efficacy of IV lignocaine in 50 patients with fibromyalgia.
Intravenous lignocaine appears to be both safe and of benefit in improving pain and quality of life for patients with fibromyalgia.
Short term analgesia is well documented with intravenous lignocaine in a variety of neuropathic pain states such as diabetes [17] and post-herpetic neuralgia [18], and some of these patients appear to derive prolonged relief from such treatment [23].

Headache Australia - Intravenous Lignocaine Infusions for Severe Chronic Daily Headache
Conclusions: Intravenous lignocaine appears to be useful in the management of severe intractable CDH and status migrainosus.
Intravenous lignocaine infusions have been proposed by Professor Jim Lance, as therapy for prolonged, severe migraine ("status migrainosus") but there are no published reports of use of lignocaine specifically for rebound headache (although some of the patients with CDH described by Dr. Kaube may have had rebound headache).
Raised potassium level (to 6.4mmol/L) - lignocaine infusion was stopped and potassium level remained elevated, possibly due to coexisting pyelonephritis.

 MCPC - Anaesthesia and Analgesia
Local anaesthesia (lignocaine with or without adrenaline) is used to infiltrate tissue and block the sensory nerves.
Lignocaine preparations are usually 2% or 1% and require dilution before use (Box C-1).
For an adult, this is 4 mg/kg body weight of lignocaine without adrenaline and 7 mg/kg body weight of lignocaine with adrenaline.

Save the Prepuce: Painless Separation of Preputial Adhesions
A simple technique using EMLA cream (eutectic mixture of lignocaine and prilocaine) has been devised which allows the adhesions to be separated painlessly in the outpatient clinic.

Plain and alkalinized lignocaine given directly before intubation reduced the sympathomimetic stress response resulting in a lower noradrenaline concentration and a reduced heart rate.

Comparative study of centbucridine and lignocaine for subarachnoid block. Dasgupta DD, Tendulkar BA, Paul TT, Satoskar ...
As regards overall assessment, 65 patients in the centbucridine group and 68 patients in the lignocaine group were rated as good; 7 in the centbucridine group and 3 in the lignocaine group as 'fair' while 5 in the centbucridine group and 4 in the lignocaine group were classified as 'poor' responders.
Fourteen patients in the lignocaine group complained of backache as compared to 5 in the centbucridine group.
These results confirm the earlier findings[1],[6] that given by spinal route; cent bucridine 1% produces anesthetic effect comparable to 5% lignocaine and hence, can be used as a substitute for lignocaine.

It is well absorbed from mucous membranes and is therefore used as a surface anaesthetic at concentrations of 2-4%.
Pregnancy – As with the use of most drugs the benefit of using lignocaine must clearly outweigh the risks to the foetus especially in early pregnancy.

To determine the effect of single doses of nebulised lignocaine (80 mg and 160 mg) on FEV1, bronchial reactivity, heart rate, blood pressure and plasma lignocaine levels over 2 hours in patients with relatively mild asthma
To determine whether any tendency to bronchoconstriction with nebulised lignocaine can be prevented by pre-treatment with salbutamol.
Future work: if nebulised lignocaine can be confirmed to be safe and well tolerated either with or without pre-treatment with salbutamol the researchers intend to proceed with studies of regular treatment in subjects with more severe asthma to determine its steroid sparing effects.
