 Drug overdose Summary
Accidental overdose is common among users of illegal substances of abuse, since little reliability can be placed on the potency, presence of adulterants, and even identity of the street substance.
Obtaining a careful drug history from the patient or accompanying individuals is of paramount importance in effectively treating and minimizing risks from a drug overdose, which often results in death.

A drug overdose occurs when a drug is ingested in quantities and/or concentrations large enough to overwhelm the homeostasis of a living organism, causing severe illness or death.
The word "overdose" implies that there is a safe dosage and therefore the term overdose is commonly only applied to drugs, not poisons.
Drug overdoses are sometimes caused intentionally to commit suicide, but many drug overdoses are accidental and are usually the result of either irresponsible behavior, or the misreading of product labels.

Although drug abuse in and of itself carries with it an inherent risk of overdose and death, engaging in risky or reckless behavior is not generally considered sufficient evidence of suicidal intent.
Accidental deaths thus comprise something of a composite category; deaths under this rubric may be the result of a range of heterogeneous drug uses.
Drug deaths have occurred in every county of the state in proportions roughly equivalent to the population as a whole.

Potential Interactions with Drugs that Inhibit or are Metabolized by Cytochrome P450 Isozymes: Multiple hepatic cytochrome P450 (CYP450) enzymes are involved in the oxidative biotransformation of a large number of structurally different drugs and endogenous compounds.
A clinically significant fluvoxamine interaction is possible with drugs having a narrow, therapeutic ratio such as terfenadine, astemizole, cisapride, or pimozide, warfarin, theophylline, certain benzodiazepines and phenytoin.
Epidemiological studies of the case-control and cohort design that have demonstrated an association between use of pyschotropic drugs that interfere with serotonin reuptake and the occurrence of upper gastrointestinal bleeding have also shown that concurrent use of an NSAID or aspirin potentiated the risk of bleeding.

Consequently, patients should be evaluated carefully for a history of drug abuse, and such patients should be observed closely for signs of misuse or abuse of olanzapine (e.g., development of tolerance, increases in dose, drug-seeking behavior).

Since DAWN covers about one-third of the nation's population but almost all major urban areas where drug use florishes, totals were doubled to arrive at yearly estimates of 2,000 for heroin deaths and 1,000 for cocaine deaths.

Many of the emergency room visits and deaths linked to acetaminophen poisoning are due to accidental or intentional overdoses (for example, suicide attempts).
Alcohol and many other drugs also use the cytochrome P450 processing system, and the risk of a “bottleneck” is greater if the liver has to deal with both acetaminophen and these other substances at the same time.
This is important because unintentional overdoses can occur when people take two or more medications together without realizing they all contain acetaminophen.

Approximately one-fifth of these deaths [60-80] are considered to be accidental.” Thus in those two countries alone, with a population of 53 million people---approximately 18% of the size of the U.S.-- there were an estimated 60 to 80 accidental deaths a year from co-proxamol or propoxyphene/acetaminophen.
First, because of the nature of Federal reporting, there are no data categorized by accidental deaths either for the first two decades of propoxyphene use prior to 1981 or for the years since 1999.
That this drug, which has been associated with at least 7109 reported deaths including 2110 in which the death was accidental and many times more emergency room visits since September 1972, is a serious public health problem is not disputable.  Only by banning propoxyphene can this danger be eliminated.

We list this drug as a Do Not Use drug because it is no more effective than aspirin and is addictive.
These two drugs work in different ways, and when they are used together, they generally relieve pain that would otherwise require a higher dose of narcotic, while causing fewer adverse effects.
The decision to remove this drug from the market is based on the fact that it is a poor pain reliever and that the risk of toxicity in overdose, both accidental and deliberate, is unacceptable.
