In Brief: The Joint Commission’s list of dangerous abbreviations, acronyms, and symbols not to be used became effective on January 1, 2004.
To help reduce the numbers of medical errors related to incorrect use of terminology, the Joint Commission on Accreditation of Healthcare Organizations (JCAHO) recently issued a list of abbreviations, acronyms, and symbols that should no longer be used.
Medication orders are subject to the initiative, but so is all clinical documentation, including orders, progress notes, consultation reports, and operative reports, as well as educational materials and protocols/pathways.

Although once the acronym is regularly pronounced, it is subject to syllabic acronymizing, as in the 'you-dub' pronunciation of the orthographic acronym (UW) for the University of Washington.
Medical people (at least in the New York area) seem to usually pronounce OB-GYN letter-name by letter-name so that it has five stressed syllables, rather than saying either (1) "obstetrician-gynecologist" or "obstretrics and gynecology" (3 to 5 main or secondary stresses) or (2) /ab gayn/ (2 stresses).

Inspired by an exhaustive list of medical trial acronyms, last year I wrote wrote a post that still makes me smile (the part where my mind's eye conjures Batman at a medical conference).
As compared with studies without acronym names, acronym-named studies had higher Jadad methodologic quality scores, enrolled five times as many patients, had follow-up periods half as long, but were not more likely to report positive results.

2001 APR 9 - (NewsRx.com and NewsRx.net) -- Adverse events in surgery are the most frequent and costly type of medical error occurring in hospitals, according to statistics from the National Patient Safety Benchmarking Center, a repository of data on adverse events in the U.S. collected and analyzed by Safety-Centered Solutions, Inc. (SCS).
Adverse events in surgery account for nearly 20% of events in the national database; medication errors make up 16%; medical (nonsurgical treatment) account for 14.8%; patient falls make up 8.8%; and nosocomial infections round out the top five most frequent adverse events with 7.5%.
According to SCS' National Patient Safety Benchmarking Center, the five most costly adverse events are: surgical errors, comprising 31% of the costs; medical (nonsurgical treatment), 16.6%; nosocomial infections, 13.8%; medication errors, 11.7%; and pressure ulcers, 8.3%.
