Telephone switchboard - Wikipedia, the free encyclopedia
When calling long distance, the customer often would not have the phone number available, so would simply ask for the name and city of the person desired.
The long distance operator would plug into the trunk for the distant city, and the inward operator in the distant city would answer, obtain the number from their local information operator, and ring the call.
If the number was in a distant city, the operator would call the inward operator in the destination city, and ask her to try the number, or to test a line to see if it was busy or out of order.

Long distance operator-assisted rates from pay phones are often high, sometimes set at the maximum rate allowed under current regulations.

The introductory words ("long distance operator with a collect call from...") are said quickly and are glossed over, and normally the party called doesn't listen with sufficient care to determine that "long distance operator" isn't preceded with a company name.
Further, the dialogue reveals that the initial words were "Long distance operator" and not "This is the long distance operator," which removes the second or so that listeners require to get their audio bearings that an extra word or two would provide.
For GTE to use this excuse to distance themselves from their obligation to assure that they do not promote rip-off by serving as a collection agent for those ripping-off is merely another form of denying both the problem and their obligation to investigate complaints for which there is evidence of deception.

 Dialing rules overview
The default rules for dialing local, long distance, and international calls are determined by the country or region that you have selected.
If you use a different long distance operator than the country/region default for all area codes, it is much easier to use a calling card for the location instead of creating an area code rule for every area or city code that you dial.
The long distance carrier number, also called the long distance operator, is dialed before the area code or country/region code to dial domestic or international long distance calls.

In the first phase, the operator would have to cover 15 per cent of the prescribed area in two years, failing which the bank guarantee, worth Rs 1 billion, would be forfeited, Paswan said.

I began noticing that if I made a call through the Verizon Long Distance operator to 800-673-7286, I could place calls to 800 numbers NOT on the AT&T network, but that the ANI was being sent as '615-986-9873' or ANI II Pair 23 followed by areacode 904.
So when I was making a call through the Verizon Long Distance operator to 800 673 7286 I would get routed to the Florida AT&T center because the Verizon Long Distance operator I got was based out of Florida(813), which is why when I had the AT&T operator dial an ANAC it would show 23-904(Florida).
However, not all Verizon Long Distance operators are based in Florida, some of them are based out of Kentucky(606) which for whatever reason will get you the Nashville, TN Center.
