Lymphoma - Wikipedia, the free encyclopedia
Lymphoma is a variety of cancer that originates in lymphocytes or, more rarely, of histiocytes.
Lymphomas are part of the broad group of diseases called hematological neoplasms.
Traditionally, Lymphoma is classified as Hodgkin's lymphoma, discovered by Thomas Hodgkin in 1832, and non-Hodgkin's lymphoma (all other types of lymphoma).

Stage II non-Hodgkin's lymphoma means involvement of two or more lymph node regions on the same side of the diaphragm (II) or localized involvement of a single associated extralymphatic organ or site and its regional lymph nodes with or without other lymph node regions on the same side of the diaphragm (IIE).
The treatment of acquired immunodeficiency syndrome (AIDS)-related lymphomas presents the challenge of integrating therapy appropriate for the stage and histologic subset of malignant lymphoma with the limitations imposed by HIV infection, to date an incurable illness.
A large number of retrospective studies and several prospective studies have been reported to use regimens such as the combination of cyclophosphamide, doxorubicin, vincristine, and prednisone (CHOP) and the combination of intermediate-dose methotrexate, bleomycin, doxorubicin, cyclophosphamide, vincristine, and dexamethasone (m-BACOD), as commonly used with non-AIDS aggressive lymphomas.

Thirty-seven lymphomas of bone were studied, including 33 diffuse large cell lymphomas, three undifferentiated (small noncleaved cell) lymphomas, and one well-differentiated (small) lymphocytic lymphoma.
The large cell lymphomas were subclassified as large cleaved, large noncleaved, multilobated cell, and immunoblastic sarcoma (large cell lymphoma, immunoblastic type).
Sixty-seven percent of patients with large cleaved and multilobated cell lymphoma were long-term survivors, but only 21% of those with large noncleaved cell and immunoblastic sarcoma were.

 Lymphoma AIDS-Related Lymphoma
The incidence of non-Hodgkin's lymphoma has increased in an almost parallel course with the AIDS epidemic and accounts for 2% to 3% of newly diagnosed AIDS cases.[2] Pathologically, AIDS-related lymphomas are comprised of a narrow spectrum of histologic types consisting almost exclusively of B-cell tumors of aggressive type.
The diagnosis of AIDS precedes the onset of non-Hodgkin's lymphoma in approximately 57% of the patients, but in 30%, the diagnosis of AIDS is made at the time of the diagnosis of non-Hodgkin's lymphoma and HIV positivity.[7] The geographic distribution of these lymphomas is also similar to the geographic spread of AIDS.
The EBV genome has been detected in varying numbers of patients with AIDS-related lymphomas; molecular analysis suggests that the cells were infected before clonal proliferation began.[1] EBV is detected in 30% of patients with small, non-cleaved and in 80% of patients with diffuse large cell lymphomas.

 Large cell lymphomas
Large Cell Lymphomas (LCL) are one of the most common types of aggressive lymphoma, comprising about 31% of lymphomas.
Briefly, lymphomas result when DNA damage or changes occurs to an immune cell (a lymphocyte) that alters the behavior of the cells.
These malignant cells then may accumulate to form tumors that may enlarge the lymph nodes or spread to other areas of the lymphatic system, such as the spleen or bone marrow.

Primary Large B-cell Lymphoma of the Skin
Although primary cutaneous diffuse large B-cell lymphomas (DLBCLs) except for those of the leg are grouped together with primary cutaneous follicle center cell lymphoma in the European Organization for Research and Treatment of Cancer classification of primary cutaneous lymphomas, they typically lack the usual phenotypic profile of follicular lymphoma.
Among the cutaneous B lymphomas, B-cell lymphomas of the lower limbs appear as a special subgroup with a prognosis that is possibly worse than that of primary cutaneous B-cell lymphomas located on the trunk, arms or head, with more frequent relapses.
With a mean follow-up of 31 months, the study confirmed the worse prognosis of lymphomas with a predominance of centroblasts and immunoblasts (3 deaths) compared with lymphomas with a predominance of centrocytes, as well as their higher rate of recurrence (7/11).

Most cases of follicular lymphoma, especially those rich in small-cleaved cells, have a t(14;18) gene translocation.
Many cases of small non-cleaved cell lymphomas including Burkitt's also have a gene translocation - the MYC gene, t(8;14).

eMedicine - Lymphoma, High-Grade Malignant Immunoblastic : Article Excerpt by: Beata Holkova, MD
Background: Immunoblastic lymphoma (IBL), also known as diffuse histiocytic lymphoma, is a malignant disorder of the B cell.
In the Working Formulation (an international classification system, 1982), IBL is classified with lymphoblastic and small noncleaved cell lymphoma as a high-grade non-Hodgkin lymphoma (NHL).
Although initially IBL was considered a more aggressive variety of lymphoma with a poor prognosis when compared to diffuse large cell NHL (DLCL), with newer combination chemotherapeutic approaches it is now associated with a similar overall survival rate when compared to DLCL.

 AIDS-related lymphoma
Since 1984, several series of cases of Hodgkin's lymphoma occurring in patients at risk for AIDS have been reviewed.[14] However, Hodgkin's lymphoma is still not part of the CDC definition of AIDS because there has been no clear demonstration of its increased incidence in conjunction with HIV, as is the case for aggressive non-Hodgkin's lymphoma.
HIV-associated Hodgkin's lymphoma presents in an aggressive fashion, often with extranodal or bone marrow involvement.[14] A distinctive feature of HIV-associated Hodgkin's lymphoma is the lower frequency of mediastinal adenopathy compared to non-HIV-associated Hodgkin's lymphoma.
