As the dead and injured were removed from the collision of Amtrak's Capitol Limited and MARC commuter train 286, something became painfully clear to the officials who first walked the smoky scene: The MARC train was somewhere it shouldn't have been.
The commuter train should have stopped at a signal several hundred feet from the point of impact, but instead struck the side of the Amtrak locomotives as the Capitol Limited moved through a switch to cross over to an adjacent track.
Trains may run in either direction on either track at Silver Spring, protected by signals that remain red until the dispatcher clears a train to pass.

MARC, prior to 1984 known as Maryland Rail Commuter Service, is a commuter rail system comprising three lines in the Baltimore-Washington Metropolitan Area.
MARC is administered by the Maryland Transit Administration (MTA), a Maryland Department of Transportation agency, and is operated under contract with CSX Transportation and the National Railroad Passenger Corporation (Amtrak).
On February 16, 1996, during the Friday-evening rush hour, a MARC bound for Union Station collided with an outgoing Amtrak train and erupted in flames on a snow-swept stretch of track in Silver Spring, Maryland.

MARC accident, Silver Spring, Md. On February 16, 1996, at approximately 5:40 p.m., a near-head-on collision occurred between Maryland Rail Commuter Authority (MARC) train P28616 and National Railroad Passenger Corporation (Amtrak) Train PO2916 on the CSX Transportation line at Silver Spring, Maryland (milepost 8.3).
The MARC train consisted of a cab control car in the lead followed by two passenger coaches and a locomotive pushing the consist.
The MARC train proceeded past the signal and struck the midpoint of the lead locomotive of the Amtrak train, which was diverging from Track No. 2 to Track No. 1 through the crossover.

If the train continues to exceed that speed within a set limit of time the train brakes are automatically initiated bringing it to a stop.
The BNSF train, which became a runaway when it lost braking power, was cabooseless, and its telemetry device, like those on many trains, was not "armed" with a two-way braking system.

The MARC operator should have seen a yellow caution light about three miles before the crash site, indicating that there was a red signal ahead that would require him to stop, Goglia said.
Crew members from another train said they heard the MARC train operator acknowledge seeing the signal, but they don't recall what he said it was, Goglia said.

As MARC train 286 approached Kensington the colour-light signal displayed a yellow aspect, warning that the next signal was at red.
The locomotives of the Amtrak train were on the crossover between the southbound and northbound lines when the collision occurred.
It also highlighted the lack of some form of Automatic Train Control (ATC) which would have given the driver a positive indication that his actions were contrary to the authorised movement of the Amtrak train.

Eleven passengers on the MARC train, which was traveling to Washington's Union Station, and 15 of the 182 crew members and passengers on Amtrak's 'Capitol Limited,' on a run from the nation's capital to Chicago, were injured.
The NTSB said the probable cause of the accident was the apparent failure of the MARC train engineer and the train crew, because of multiple distractions, to obey a signal that required the train to proceed at a slow speed after a station stop and be prepared to stop at the next signal.
All crew members on both trains were in good health, had no evidence of fatigue, and were experienced in and qualified for their duties.

To accomodate the trains in Martinsburg, railroad crews had to rebuild two spur tracks near the freight station and shift the main line, also known as the Capitol Limited for Amtrak passenger service.
Trains travel about 60 and 65 miles per hour, but now are sometimes slowed by short crossovers between tracks.

The commuter train agency, a part of the Maryland Transit Administration, said the reliability of these engines has been "less than ideal for both Amtrak and MARC." The locomotives, similar to those used on Amtrak's Acela train, are made by a consortium of the Canadian firm Bombardier and France's Alstom.
Trains have been delayed, and sometimes canceled, as a result of freight congestion, switch problems, equipment failure, track inspections and construction projects.

MARC reports that train 874 has finally made it around a stopped CSX freight train.
According to the MARC Web site, Train 874 will now operate as train 880 out of Brunswick, Md., and it will make all stops on its way to D.C. And according to MARC's Website, train 876's equipment will leave Brunswick at 607 a.m., train 874's normal leaving time.
Train 876 may use the equipment of train 880 from Brunswick, but will depart late.

Current NTSB reports fault the engineer of the MARC train with failing to remember a yellow signal (half speed = 30 mph) that was displayed prior to reaching a drop off station.
