MPlayer uses FFmpeg (libavcodec) to decode the video stream itself, but has its own demuxers/splitters for handling the file containers (examples of which are AVI, MOV, OGM, MKV, FLV, etc).
MPlayer is however slowly moving towards replacing its custom demuxers/splitters with the ones from FFmpeg's "libavformat" library so that the work of the two projects can be combined.
XBMC uses MPlayer as our main 'core' to play video/audio files and therefore XBMC only support those codecs which MPlayer can demux and decode, (well, most of the media codecs/formats that MPlayer supports anyway), and MPlayer in turn get most of its codec support from the FFmpeg codec-suit.

I'm pretty sure there are a few gurus here who have managed to compile mplayer on os X.4 and probably keep updated cvs on their systems.
Mplayer osX has similar chops, and the mplayer mac preview on the downloads page you pointed me to, though the pans are smooth, fails to properly render h.264 clearly, producing macroblocks and noise which remind me of.asx files from the dark ages of fall 2001.
CVS MPlayer is not fully functional without a copy of the libavcodec and libavutil libraries from FFmpeg.

I've run mplayer on a variety of machines and I've never had to alter kernel settings to get smooth playback on any system that had the cpu power to decode the files i was using.
MPlayer is definitely the best movie player out there, but at this point in time, it's gui is pretty pathetic, with stupid behaviour like the one you describe.

My mplayer works best when audio is set to alsa and video is set to X11.
what this means (as far as i'm aware) is that the rpm of mplayer that you installed was not compiled with the necessary codec installed for that particular file.
